中晚期口腔鳞状细胞癌术前多西他赛联合顺铂及5-氟尿嘧啶诱导化疗的安全性研究

摘　　要：目的观察多西他赛联合顺铂及5-氟尿嘧啶（TPF）术前诱导化疗治疗中晚期口腔鳞状细胞癌的安全性。方法46例中晚期口腔鳞状细胞癌患者,其中观察组23例术前给予TPF方案诱导化疗,7~10 d后手术治疗;对照组23例单纯给予手术治疗。观察诱导化疗的不良反应及两组术后并发症发生率。结果 TPF诱导化疗的不良反应主要为骨髓抑制、胃肠道反应,患者可耐受。两组术后均并发切口感染、出血、乳糜瘘等,观察组术后并发症发生率为13.0%（3/23）,对照组为17.39%（4/23）,两组比较差异无统计学意义（P〉0.05）。结论中晚期口腔鳞状细胞癌患者术前TPF诱导化疗,不良反应较轻,对术后并发症无明显影响。Objective To observe the safety of preoperative induction chemotherapy with docetaxel,cisplatin and 5-fluorouracil（ TPF） for middle or late stage oral squamous cell carcinoma. Methods Forty-six patients with middle or late stage oral squamous cell carcinoma were enrolled. Twenty-three patients in the observation group received the preoperative induction chemotherapy with TPF,and then underwent surgery 7 to 10 days later. Twenty-three patients in the control group underwent surgery alone. The adverse reactions of induction chemotherapy as well as the incidences of postoperative complications in both groups were observed. Results The main adverse reactions of induction chemotherapy with TPF were myelosuppression and gastrointestinal reaction. And all patients in the observation group were tolerant of the adverse reactions. Incision infection,bleeding and chylous fistula occurred after surgery in both groups. The incidences of postoperative complications in the observation group and control group were 13. 0%（ 3 /23） and 17. 39%（ 4 /23）,respectively,and there was no significant difference between two groups（ P 〉 0. 05）. Conclusion For patients with middle or late stage oral squamous cell carcinoma,preoperative induction chemotherapy with TPF causes mild adverse reactions and has no obvious effect on postoperative complications.
