摘 要:目的探讨人乳头瘤病毒E6/E7 mRNA表达水平与宫颈病变病理级别的相关性。方法选取宫颈病变患者500例,收集宫颈口及宫颈管的脱落细胞行E6/E7 mRNA定量检测,阴道镜下宫颈多点取活组织病理检查,根据病理学结果明确诊断并进行分组,分析E6/E7 mRNA表达量与宫颈病变病理级别的关系。结果共228例患者E6/E7 mRNA阳性,检出率为45.6%。在炎症组、宫颈上皮内瘤变(CIN)组及癌组中,E6/E7 mRNA阳性检出率随组织学诊断的严重程度增高而升高(P<0.05)。宫颈高级别组(≥CINⅡ)E6/E7 mRNA阳性检出率为78.75%,明显高于宫颈低级别组(≤CINⅠ)的24.76%(P<0.05)。不同病理诊断组间E6/E7 mRNA基因表达量比较,差异有统计学意义(P<0.05),E6/E7 mRNA基因表达量与宫颈病变病理级别呈正相关(P<0.05)。结论 E6/E7 mRNA癌基因表达量随着宫颈病变病理级别升高而升高。Objective To explore the correlation between the expression level of human papillomavirus( HPV) E6 /E7 mRNA and pathological grades of cervical lesions. Methods A total of 500 patients with cervical lesions were enrolled. The cast-off cells from cervical orifice and canals were collected for the quantitative detection of E6 / E7 mRNA. Multiple punch biopsy of cervix was performed under colposcope. The patients were grouped according to the pathological results. The relationship between the expression level of E6 / E7 mRNA and pathological grades of cervical lesions was analyzed. Results E6 / E7 mRNA was positive in 228 cases,and the detective rate was45. 6%. In inflammation group,cervical intraepithelial neoplasia( CIN) group and cancer group,the detective rate of E6 / E7 mRNA increased with the increasing histological severity( P 〈 0. 05). The detective rate of E6 / E7 mRNA in the high-grade cervical lesions( ≥CIN Ⅱ) group was significantly higher than that in the low-grade cervical lesions( ≤CINⅠ) group( 78. 75% vs. 24. 76%,P 〈 0. 05).There was significant difference in the expression level of E6 / E7 mRNA among cervical lesions with different pathological grades( P 〈 0. 05). The expression level of E6 / E7 mRNA positively correlated with pathological grades ofcervical lesions( P 〈 0. 05). Conclusion The expression level of E6 / E7 mRNA increases with the increasing pathological grades of cervical lesions.
