常规超声结合弹性成像对非哺乳期乳腺炎与乳腺癌的鉴别诊断价值

摘　　要：目的探讨常规超声（US）结合超声弹性成像（UE）对非哺乳期乳腺炎（NLM）与乳腺癌的鉴别诊断价值。方法回顾性分析48例非哺乳期乳腺炎和360例乳腺癌患者的乳腺超声表现。结果NLM组病灶后方回声衰减、微钙化、Ⅱ~Ⅲ级血流、RI〈0.70及UE评分1~3分与乳腺癌组比较,差异有统计学意义（P〈0.05）。结论非哺乳期乳腺炎性肿块多累及脂肪层,伴有周围组织水肿,部分呈“倒三角形”低回声,病灶内出现斑点状或小条状钙化斑,血流信号丰富,RI〈0.70,UE评分1~3分,上述超声表现对非哺乳期乳腺炎与乳腺癌的鉴别诊断有一定参考意义。Objective To explore the differential diagnosis value of conventional ultrasonography combined with ultrasonic elastography for non-lactation mastitis and breast cancer. Methods The breast ultrasound performance of 48 non-lactation mastitis and 360 breast cancer were analyzed retrospectively. Results There were significant differences in decreased posterior echoes, microcalcifications, blood flow rating Ⅱ-Ⅲ in lesion, RI〈0. 70 and UE score 1-3 shown on ultrasonic images between NLM and breast cancer（P〈0; 05）. Conclusions The ultrasonic images of NLM lesions were characterized by invaded the subcutaneous fat layer mostly, around tissue occured edematous, shown as ＂inverted triangle＂ hypoecho partly, appeared punctate or small strip calcified plaque within lesions, marked vascularity in lesions, RI〈0.70 and UE scores 1-3, which were helpful for differential diagnosis between NLM and breast cancer.
