结直肠癌放疗前后存活蛋白的表达与放疗敏感性的相关性研究

摘　　要：目的探讨结直肠癌放疗前存活蛋白表达与放疗敏感性的关系。方法选择2009年1月至2012年6月第三军医大学大坪医院野战外科研究所肿瘤中心收治的未行手术治疗的结直肠癌患者104例作为研究对象,对所有患者实施放疗,并检测放疗前后存活蛋白表达水平,分析存活蛋白与临床病理特征及放疗疗效相关性。结果放疗前和放疗后,不同年龄、性别、组织学类型、Duck分期的结直肠癌患者存活蛋白阳性表达率比较差异均无统计学意义(P>0.05);肿瘤直径<5 cm的存活蛋白阳性率高于≥5 cm者,结肠部位的阳性率高于直肠部位者,浸润程度突破肌层的阳性率高于肌层以下者,差异均有统计学意义(P<0.05);随着放疗前存活蛋白阳性表达强度的增强,结直肠癌放疗效果越差(P<0.05)。结论存活蛋白可能在结直肠癌的发生、发展中起重要作用,可作为预测结直肠预后及结直肠癌是否需要辅助放疗或改进放疗方法的指标。Objective To investigate the relationship between the expression of survivin in colorectal cancer before radiotherapy and the radiotherapy sensitivity. Methods A total of 104 colorectal cancer patients admitted to Cancer Center of Daping Hospital Research Institute of Surgery of the Third Military Medical University from Jan. 2009 to Jun. 2012 without operation treatment were included as the research objects,and radiotherapy was given to all patients,the expression level of survivin was detected before and after radiotherapy,and correlation analysis of survivin and clinical pathological characteristics and the effect of radiotherapy was done. Results There was no significant differences in livin expression positive rate between different ages,genders,tumor sizes,tumor locations,histological types,extents of invasion,Duck stages of the patients with colorectal cancer before radiotherapy（ P〉0. 05）; Positive rate of survival protein in patients with tumor diameter 〈5 cm was higher than the tumor diameter ≥5 cm,and the positive rate of survival protein in tumor location in the colon was higher than that of the rectum,the positive rate of survival protein in infiltration break through muscle layer was higher than that in muscle layer below,the differences were statistically significant（ P〈0. 05）. with the increase of survivin expression intensity before radiotherapy,the radiotherapy effect became worse（ P〈0. 05）. Conclusion Survivin may play an important role in the genesis and development of colorectal cancer,and can be used to predict the prognosis and judge the need for adjuvant radiotherapy or an index for improving radiotherapy.
