摘 要:目的:对X线平片、CT及MRI检查在脊椎转移瘤中的诊断价值进行分析比较。方法:选择2011年1月-2015年5月笔者所在医院收治的86例脊椎转移瘤患者的影像资料进行回顾性分析。所有患者均经穿刺或术后病理检查确诊,并接受X线平片、CT和MRI检查。结果:X线平片、CT及MRI对脊椎转移瘤的总出率分别为48.3%、73.0%、94.4%,MRI检出率显著高于其他两种,差异均有统计学意义(P<0.05)。对不同椎体部位转移瘤(颈椎、胸椎、腰椎、骶椎)的检出率比较结果显示,MRI检出率高于其他两组,差异均有统计学意义(P<0.05)。不同类型转移瘤(椎管受损、软组织肿物、病理性骨折)检出率比较,MRI显著高于其他两种(P<0.05)。结论:通过对三种方法的检出情况及应用特点进行分析后得出,三种检查方法相互间不可替代。常规X线平片可对多数的转移瘤做出诊断,CT可以发现早期病变,MRI诊断准确率最高。在经X线平片、CT发现异常而无法确诊时,再进行MRI检查,可以提出检出率,避免漏诊。Objective:To analyze and compare the diagnosis value of X-ray plain film,CT and MRI imaging in vertebral metastasis.Method:The images about 86 patients with vertebral metastasis treated from January 2011 to May 2015 in our hospital were retrospectively analyzed.All patients were confirmed by paracentesis or postoperative pathological examination and received the X-ray plain film,CT and MRI examination.Result:The detection rate of vertebral metastasis based on X-ray plain film,CT and MRI was 48.3%,73.0% and 94.4%. The detection rate of MRI was significantly higher than those of X-ray plain film and CT(P〈0.05).According to the detection results for metastasis of different vertebral parts(cervical vertebra,thoracic vertebra,lumbar vertebra,sacral vertebra),the detection rate of MRI was significantly higher than those of X-ray plain film and CT(P〈0.05).According to the detection results of different types of metastasis(spinal canal fracture,soft tissue mass,pathological fracture),the detection rate of MRI was significantly higher than those of X-ray plain film and CT(P0.05).Conclusion:According to the analysis results of detection rate and applications features,three methods are irreplaceable.The conventional X-ray plain film is applied for the major diagnosis of metastasis.CT is applied for the diagnosis of early lesions.MRI has the highest detection rate.When the abnormality is found but cannot be confirmed through X-ray plain film and CT,MRI can be applied to avoid the occurrence of missed diagnosis.
