摘 要:目的研究血清电解质及尿比重、p H值与上尿路结石的相关性,初步探讨高校男性青年教师结石的发病相关因素。方法回顾性分析2006年7月-2014年10月因上尿路结石住院治疗的153例高校男性青年教师临床资料,另选206例健康体检资料作为对照,比较两者血清钾(K)、钠(Na)、氯(Cl)、钙(Ca)、二氧化碳结合力(CO2-CP)、尿比重(SG)和p H值的差异,并构建Logistic回归方程。结果与对照组相比,结石组血清钾和尿p H值明显偏低,而血清钠、氯、钙和尿比重明显偏高。经过Logistic回归后得到高校男性青年教师上尿路结石的风险方程Y=exp(-115.05-2.53×K+0.29×Na+5.20×Ca-1.14×p H+74.69×SG)/[1+exp(-115.05-2.53×K+0.29×Na+5.20×Ca-1.14×p H+74.69×SG)]。结论血清钾、钠、钙、p H值和尿比重可能参与上尿路结石的发生过程。风险方程的应用有助于该疾病的个体化防治。Objective To explore risk factors of upper urinary tract lithiasis in young male university teachers by assessing the relevance between serum electrolyte, urine specific gravity(SG), urine p H and the development of lithiasis. Methods Between July2006 and October 2014, a total of 153 young male university teachers suffered from upper urinary tract lithiasis were involved in this research. Other 206 healthy young male university teachers were considered as control. Serum levels of K, Na, Cl, Ca, CO2-CP,urine p H and SG were analyzed between the two groups. The Logistic regression equation was established. Results There were significant differences in levels of serum K, Na, Cl, Ca, urine p H, and SG. Logistic regression analyses showed that K and urine p H were protective factors, while Na, Ca, and SG were risk factors for upper urinary tract lithiasis in young male university teachers. The risk equation of urinary stones was established by the Logistic regression: Y=exp(-115.05-2.53×K+0.29×Na+5.20×Ca-1.14×p H+74.69×SG) /[1+exp(-115.05-2.53×K+0.29×Na+5.20×Ca-1.14×p H+74.69×SG) ]. Conclusion Serum K, Na, Cl, Ca,urine p H, and SG may affect the pathogenesis of upper urinary tract lithiasis, and the use of risk equation may contribute to preventing and curing this disease with individualized pattern.
