郴州市9521例0～3岁婴幼儿肥胖率调查分析

摘　　要：目的调查郴州市婴幼儿的体格发育状况,了解郴州市0~3岁婴幼儿的肥胖发生率,为今后积极开展干预措施、制订婴幼儿营养及健康保健方案提供参考依据。方法选择2012年1月至2013年12月郴州市第一人民医院儿童保健科门诊就诊的0~3岁婴幼儿共9 521例,按年龄分为15组;按地域分城区和农村两组。测量所有婴幼儿体质量、身长,并计算出身长别体质量的Z值（WLZ）,统计分析各组婴幼儿肥胖发生率及总肥胖发病率的性别与城乡差异。结果各年龄段肥胖率以6月龄最高,其中城区男、女婴幼儿肥胖率分别在6~〈8、5~〈6个月组达高峰（6.86%、5.56%）;农村男、女婴幼儿肥胖率在3~〈4、4~〈5个月组达高峰（4.35%、4.83%）。9 521例婴幼儿总肥胖发病率为2.91%,其中男性为2.89%,女性为2.92%,二者比较,差异无统计学意义（P〉0.05）。城区婴幼儿肥胖发病率为3.23%,农村婴幼儿为2.53%,二者比较,差异有统计学意义（P〈0.05）。结论郴州市0~3岁婴幼儿的肥胖率在6月龄最高,总肥胖率存在城乡差异,性别差异不明显。Objective To investigate the physical development status of infants and young children in Chenzhou City and to understand the obesity occurrence rate in infants and young children aged 0-3 years old in order to provide a reference ba- sis for actively carrying out the interventional measures and formulating the nutrition and healthcare scheme of infants and young children in the future. Methods A total of 9 521 infants and young children aged 0-3 years old in the child healthcare depart- meat of our hospital from January 2012 to December 2013 were selected and divided into 15 groups according to age ,meanwhile also divided into the urban group and rural group according to the region. The body weight and body length were measured in all the children, the Z value of the body length to the body mass was also calculated. The obesity rates in various age groups were statistically analyzed. The urban and rural difference and gender difference in the total obesity rate was also analyzed. Results Among various age groups, the 6-month group had the highest obesity rate. The obesity rate of boys and girls in the urban area reached the peak in the 6-month old and 5-month old groups （6.86%, 5.56%）, respectively; while those in the rural area reached to peak in the 3-month old and 4-month old groups （4.35% ,4.83%）, respectively;the total obesity rate in 9521 infants and young children was 2.91%, 2.89% in boys and 2.92% in girls, without statistically significant difference between them （P〉0.05）. The incidence rate of obesity was 3.23% in urban area and 2.53% in rural area,the difference was statistically significant（P〈0.05）. Conclusion The obesity rate of young children aged 0-3 years old in Chenzhou is highest in 6-month old. The urban and rural difference exists in the total obesity rate ,but the gender difference is not obvious.
