Ex-PRESS青光眼引流器植入术与小梁切除术治疗外伤性青光眼的效果比较

摘　　要：目的探讨Ex-PRESS青光眼引流器植入术与小梁切除术治疗外伤性青光眼的疗效。方法采用前瞻性随机对照临床试验研究,选择外伤性青光眼40眼,随机分为两组。Ex-PRESS青光眼引流器植入术组(A组,21眼)和小梁切除术组(B组,19眼),末次随访时间术后1年,分别对两组术后视力、眼压、成功率进行比较。结果 A组患者治疗后视力改善率明显高于B组(P<0.05),A组和B组患者术后眼压较术前明显下降(P<0.05),而远期随访A组术后平均眼压明显低于B组(P<0.05),A组和B组术后绝对成功率和失败率比较,差异有统计学意义(P<0.05),A组绝对成功率明显高于B组,A组失败率明显低于B组。结论 Ex-PRESS青光眼引流器植入术治疗外伤性青光眼相对简便、安全、微创、学习曲线短,成功率高,值得临床推广使用。Objective To explore the efficiency of trabeculectomy and Ex-PRESS glaucoma filtration in patients suffered ocular traumatic secondary glaucoma. Methods Use prospective randomized controlled clinical trials choose 40 eyes of traumatic glaucoma were randomly divided into two groups.All patients were assigned to groups A and B according the different therapy.Group A（21 eyes） was treated with Ex-PRESS glaucoma filtration device implantation.Group B（19 eyes） was treated with trabeculectomy.The last follow-up time was 1 year after surge.The postoperative visual acuity,intraocular pressure and success rate were analyzed. Results The ratio of improved visual acuity in group A was significantly higher than that of group B（P〈0.05）.Compared with that before operation,intraocular pressure both in group A and group B significantly decreased（P〈0.05）,but the average of intraocular pressure in group A was lower than that of group B during the long term follow up（P〈0.05）.Definitely sucess and fail rate was statistical difference between group A and group B（P〈0.05）.Definitely success in group A was higher than that of group B.Fail rate in group A was lower than that of group B. Conclusion Ex-PRESS glaucomafiltration device implantation has the advantages ofsimpleness,safety,minimally invasive,short learning curve for ocular traumatic secondary glaucoma,Ex-PRESS glaucoma filtration is worth clinical application.
