两种不同回肠造口方法在老年人低位直肠癌前切除术中应用的对比分析

摘　　要：目的:评价回肠插管造口在老年人低位直肠癌前切除术(LAR)中应用的临床效果。方法:回顾性分析2006年6月至2012年6月广西百色市人民医院收治的68例老年低位直肠癌进行肿瘤切除回肠造口患者的临床资料。结果:回肠末端常规造口和回肠插管造口两组在术中出血量、术后肠功能恢复时间、进食流质时间、吻合口瘘发生方面比较,差异均无统计学意义(P>0.05)。在手术时间、造口并发症、生存质量、满意度调查、住院天数、治疗费用等方面,插管组低于传统组,差异均有统计学意义(P<0.05)。结论:回肠插管造口能减少手术时间、造口并发症、住院时间和治疗费用,是一种简便、安全、有效的方法。Objective： To evaluate the clinical effect of the ileum intubation colostomy of LAR in the eldly. Methods：Retrospective investigation and analysis were carried out in 68 patients with elderly low rectal cancer ,treated by tumor resection of the ileum colostomy. Results：No differences in the intraoperative blood loss, the time of gastrointestinal function recovery, the time of eating liquid, anastomotic fistula were found between the two groups （ P 〉 0. 05 ）. There were significant differences between surgery time, colostomy complications, quality of life, satisfaction survey, hospital stays and hospitalization costs （P 〈 0.05 ）. Conelusion：The ileum intubation colostomy can reduce the operation time, colostomy complications, hospitalized time and fee. It is a simple, safe and effective method.
