摘  要:目的:研究胆管细胞癌患者Ki SS-1基因mRNA的表达和血清Kisspeptin的水平,并分析其与胆管细胞癌临床病理参数的关系。方法:选择2010年1月至2014年6月期间行胆管细胞癌手术切除的患者32例,同时收集胆管细胞癌患者癌旁组织,以GAPDH基因为参考基因,采用实时荧光定量PCR检测Ki SS-1基因在胆管细胞癌组织及癌旁组织的相对表达量;同时手术前抽取胆管细胞癌患者空腹肘静脉血,采用酶联免疫吸附法检测血清中Kisspeptin的水平,以10例健康体检者空腹肘静脉血Kisspeptin的水平作对照。分析Ki SS-1基因表达量及血清Kisspeptin的水平分别与胆管细胞癌临床病理参数(年龄、性别、发生部位、分化程度、分期、淋巴结转移)之间的关系。同时手术后1个月对患者血清Kisspeptin的水平进行检测。结果:Ki SS-1基因在胆管细胞癌组织的表达量2.47±0.63显著低于癌旁组织的表达量4.36±0.87,差异具有统计学意义(t=5.749,P=0.0007)。胆管细胞癌Ki SS-1mRNA的水平及血清Kisspeptin的水平与年龄、性别及发生部位均无明显关系(P>0.05),但与分化程度、TNM分期及淋巴结转移密切相关,差异具有统计学意义(P<0.05)。胆管细胞癌患者手术前血清Kisspeptin的水平为(1.25±0.16)μg/L,显著低于健康体检者血清Kisspeptin的水平(2.37±0.14)μg/L(t=22.27,P<0.0001),手术后,胆管细胞癌患者血清Kisspeptin的水平上升至(1.91±0.11)μg/L,与手术前相比差异显著(t=22.27,P<0.0001),但仍低于健康体检者Kisspeptin的水平(t=8.684,P<0.0001)。结论:胆管细胞癌中KiS S-1基因及血清Kisspeptin的水平明显降低,并与胆管细胞癌的临床病理参数密切相关,推测其可能促进胆管细胞癌的发生、发展。Objective: To study the Ki SS-1 gene expression and the level of serum Kisspeptin in patients with cholangiocarcinoma,and analyze their relationship with cholangiocarcinoma clinicopathological parameters. Methods:All 32 patients with cholangiocarcinoma performed surgical removal in hepatobiliary surgery of our hospital from January 2010 to June 2014 were selected. Meanwhile 10 cases of normal bile duct tissue were collected as control. The Ki SS-1 gene expression in cholangiocarcinoma tissue and normal bile duct tissue were tested using real-time quantitative PCR with GAPDH gene as reference gene. Kisspeptin level in serum was detected using ELISA and compared with that in health people. The relationship of Ki SS-1 gene expression and Kisspeptin level in serum with cholangiocarcinoma clinicopathological parameters( age,sex,occurrence site,degree of differentiation,node metastasis and TNM stage) were analyzed. Results: The level of Ki SS-1 gene expression in cholangiocarcinoma tissue 2. 47 ± 0. 63 was significantly lower than that in normal normal bile duct tissues( 4. 36 ± 0. 87),with statistically significant difference( t = 5. 749,P = 0. 0007). The levels of Ki SS-1mRNA and Kisspeptinin cholangiocarcinoma displayed no apparent relationship with age,sex and occurrence site( P〉0. 05). However they were closely related to the degree of differentiation,TNM stage and lymph node metastasis,indicating statistically significant difference( P〈0. 05). The level of serum Kisspeptin in patients with cholangiocarcinoma was( 1. 25 ± 0. 16) μg / L before surgery,which was significantly lower than that in healthy people( 2. 37 ± 0. 14) μg / L. After surgery,the level of serum Kisspeptin( 1. 91 ± 0. 11) μg /L was markedly increased compared with that before surgery( t = 22. 27,P〈0. 0001),but it was still lower than that in healthy people( t = 8. 684,P〈0. 0001).
