吉西他滨联合不同铂剂治疗晚期NSCLC初治老龄患者的疗效分析

摘　　要：目的对比分析吉西他滨联合不同铂剂治疗晚期非小细胞肺癌(NSCLC)初治老龄患者的疗效及安全性。方法 122例非小细胞肺癌晚期老龄患者依照随机数字表法分为研究组和对照组各61人。对照组应用吉西他滨联合顺铂化疗,研究组应用吉西他滨联合奥沙利铂化疗。对比两组近期疗效、无进展生存时间、治疗前后的体质量与KPS评分、严重不良反应发生情况。结果两组缓解率、控制率、无进展生存时间无显著差异(P>0.05)。两组治疗前体质量及KPS评分无显著差异(P>0.05),治疗后两组体质量及KPS评分较治疗前均显著改善(P<0.05),研究组体质量及KPS评分改善幅度显著优于对照组(P<0.05)。研究组严重不良反应发生率显著低于对照组(P<0.05)。结论晚期非小细胞肺癌初治老龄患者给予吉西他滨联合奥沙利铂无法提升近期疗效,但可显著提高患者的体质及生存质量,毒副反应更小。Objective To compare and analyze the efficacy and safety of gemcitabine combined with different plati-num agents in the treatment of advanced non-small cell lung cancer in early stage. Methods 122 patients with advanced non small cell lung cancer who were diagnosed and treated in our hospital from March 2012 to January 2015,cases of non small cell lung cancer patients were randomly divided into study group and control group with 61 persons in each group. Control group was treated with gemcitabine plus Cisplatin chemotherapy,research group was treated with Gemcitabine plus Oxaliplatin chemotherapy regimens. Compared the short-term efficacy,progression free survival,and the quality of the body mass and KPS score,the incidence of serious adverse reactions before and after treatment of the two groups. Results There was no significant difference between the two groups （P〉0. 05） in the remission rate,control rate and progression free survival （P〉0. 05）. There were no significant differences between the two groups before treatment （P〉0. 05）,the quality of the two groups and the KPS score were significantly improved after treatment （P〈0. 05）,the improvement of the quality of the study group and the improvement rate of KPS score were significantly better than those in the control group （P〈0. 05）. The incidence of serious adverse reactions in the study group was significantly lower than that in the control group （P〈0. 05）. Conclusion Advanced non small cell lung cancer treatment in the elderly patients given gemcitabine combined with oxaliplatin can enhance the curative effect in the near future,but can significantly improve the health and quality of life of patients,with less toxicity.
