22215个IVF/ICSI周期的不孕原因分析

摘　　要：目的研究辅助生殖技术中不孕症的病因构成及年度变化。方法回顾性分析2007年1月-2014年11月共22 215个体外受精（IVF）/胞浆内单精子注射（ICSI）周期,分析不孕症的病因构成及年度变化。结果 22 215个周期中继发性不孕占49.40%,原发性不孕占50.60%。不孕的主要原因是盆腔及输卵管因素（57.00%）,其次为双方因素（18.65%）及男方因素（14.90%）。盆腔输卵管因素在继发性不孕中所占比例（70.19%）显著高于在原发性不孕中所占比例（44.12%）（P〈0.05）。盆腔及输卵管因素不孕患者中,继发性不孕比例（60.83%）高于原发性不孕比例（39.17%）。其他不孕原因中,均是原发性不孕比例高于继发性不孕比例。2007-2014年盆腔及输卵管因素都是助孕的首要指征,占52.0%~60.1%。结论无论继发性不孕还是原发性不孕,输卵管因素都是IVF/ICSI治疗的首要指征。盆腔及输卵管因素不孕患者中,继发性不孕比例高于原发性不孕比例;其他各种不孕原因中,原发性不孕比例高于继发性不孕比例。Objective To research the causes of infertility and annual changes in in vitro fertilization（ IVF） / intracytoplasmic sperm injection（ ICSI） cycles. Methods A total of 22 215 IVF / ICSI cycles from January 2007 to November 2014 were analyzed retrospectively,the causes of infertility and annual changes were analyzed. Results Among 22 215 IVF / ICSI cycles,the proportions of secondary infertility and primary infertility were 49. 40% and 50. 60%,respectively. The main cause of infertility was pelvic-tubal factor（ 57. 00%）,followed by female-male factors（ 18. 65%） and male factor（ 14. 90%）. The percentage of pelvic-tubal factor in patients with secondary infertility was 70. 19%,which was significantly higher than that in patients with primary infertility（ 44. 12%）（ P〈0. 05）. Among the patients with pelvic-tubal factor,the percentage of secondary infertility was 60. 83%,which was higher than that of primary infertility（ 39. 17%）. Among the patients with other causes of infertility,the percentage of primary infertility was higher than that of secondary infertility. The annual percentage of pelvic-tubal factor was 52. 0%-60. 1% from 2007 to 2014,which was the primary causes of infertility each year. Conclusion Pelvic- tubal factor was the main indication of IVF / ICSI in both primary infertility and secondary infertility. Among the patients with pelvic-tubal factor,the percentage of secondary infertility was higher than that of primary infertility. Among the patients with other infertility causes,the percentage of primary infertility was higher than that of secondary infertility.
