加味当归补血汤治疗肿瘤相关性贫血72例临床观察

摘　　要：目的：对比分析加味当归补血汤与常规西药治疗肿瘤相关性贫血的临床效果,为临床治疗提供新的思路。方法：连续搜集2010年5月~2013年5入我院肿瘤科恶性贫血患者共72例,男性41例,平均（56.7±12.5）岁,将其随机分为观察组（n=40例）和对照组（n=32例）,观察组患者给予加味当归补血汤100ml,1日2次;对照组患者给予铁剂、叶酸、维生素B12,必要时输血治疗。分析比较两组患者4周后贫血指标（RBC、Hb、MCHC、总铁结合力和网织红细胞）,不良反应发生率及免疫指标（IL-2和NK细胞活性）的差异性。结果：两组患者治疗后RBC、Hb和MCHC均较前升高,总铁结合力和网织红细胞水平均较前降低,且观察组与对照组相比较,差异均有统计学意义（P〈0.05）;两组患者的不良反应总发生率相比较,观察组显著低于对照组（P〈0.05）;治疗后两组患者的IL-2和NK细胞活性相比较,观察组显著高于对照组（P〈0.05）。结论：加味当归补血汤治疗肿瘤相关性贫血的效果可能优于常规的西药治疗,值得临床推广。Objective ： To study the clinical effect of Modified Danggui Buxue Decoction for 72patients of cancer related ane- mia. Methods： A total of 72patients of cancer related anemia from May 2010to May 2013, 41males and the mean age was （56.7 ±12. 5） years of which they were randomly divided into experiment group （n=40） and controlled group （n=32） , the patients in the experiment group received Danggui Buxue Decoction 100ml twice a day, the patients in the controlled group received Iron, folie acid, vitamin B12, or blood transfusion if necessary. Then to compare the differences of anemia indicators （ including RBC, Hb, MCHC, Total iron binding capacity and reticulocytes）, complications rate and Immune index （IL-2and NK cell activity）. Results ： The levels of RBC, Hb and MCHC were higher 4weeks later in the two groups and total iron binding capacity, retieulo- cytes were lower; thus they in the experiment group were all significantly more improvements than the controlled group （P〈 0.05 ）. The complications rate in the experiment group was significantly lower （P〈0. 05）. The IL-2and NK cell activity in the two groups were higher after treatment, and they in the experiment group were both significantly higher than the controlled group （ P〈0. 05 ）. Conclusion ： The clinical effect of Modified Danggui Buxue Decoction for cancer related anemia can be more prior to common medical drugs.
