摘  要:目的探讨人类着色性干皮病基因D(XPD)多态性与吸烟的交互作用对胰腺癌易感性的影响。方法采用病例对照研究对226例胰腺癌患者和263例对照提取外周血DNA,应用SNaPshot技术对XPD基因6个单核苷酸多态性(SNP)位点(rs13181、rs3916874、rs238415、rs50872、rs50871、rs238406)进行基因分型,比较不同基因型联合吸烟与胰腺癌发病风险的关系。结果吸烟者胰腺癌的发病风险较不吸烟者增加了0.381倍(P〉0.05),每13吸烟≥20支者、吸烟≥146/年、吸烟I〉246/年、吸烟≥37影年者胰腺癌的发病风险分别增加了0.569(P〈0.05)、0.767(P〈0.05)、0.856(P〈0.05)和1.209(P〈0.05)倍。胰腺癌患者rs13181位点C等位基因频率高于对照(P〈0.01);携带至少1个突变等位基因C(AC、CC或AC+CC)的个体胰腺癌的发病风险增高(P〈0.05);携带突变等位基因C(AC+CC)的个体中,不吸烟者胰腺癌的发病风险增高了0.384倍(P〉0.05),每日吸烟≥20支者、吸烟≥14包/年、吸烟≥24包/年、吸烟≥37包/年者胰腺癌的发病风险分别增高了2.343(P〈0.01)、2.309(P〈0.05)、4.011(P〈0.05)、3.013(P〈0.05)倍。结论XPD基因rs13181位点赖氨酸-谷氨酰胺(Lys→Gln)的氨基酸替代联合吸烟对胰腺癌的发病风险可能有一定影响。Objective To investigate ihe effect of the interaction of human xeroderma pigmentosum group D (XPD) polymorphism with smoking on susceptibility to pancreatic cancer. Methods A case -control study was conducted of 226 patients with pancreatic cancer and 263 controls. Their peripheral blood DNA samples were obtained. Six single nucleotide polymorphism (SNP) loci of the XPD gene (rs13181, rs3916874, rs238415, rs50872, rs50871 and rs238406) were genotyped by SNaPshot assay for comparison of the relationship between the interaction of different genotypes with smoking and the risk of pancreatic cancer. Results The risk of pancreatic cancer had a 0. 381 - fold increase in smokers compared with non - smokers (P 〉 0. 05). The risk of pancreatic cancer in those who smoked ≥20 cigarettes per day, ≥ 14, ≥24 or ≥37 packs per year increased by 0. 569 (P 〈0.05), 0. 767 (P 〈0. 05), 0. 856 (P 〈0. 05) or 1. 209 fold (P 〈 0. 05), respectively. Patients with pancreatic cancer had higher frequencies of C allele at rs13181 than controls (P 〈0.01 ) ; the risk of pancreatic cancer increased in individuals with at least one mutant C allele ( AC, CC or AC + CC ) ( P 〈 0. 05 ) ; among individuals with mutant C allele ( AC + CC) , non - smokers had a 0. 384 - fold increase in risk of pancreatic cancer (P 〉 0. 05 ) ; The risk of pancreatic cancer in those who smoked ≥20 cigarettes per day, ≥14, 〉~24 or ≥37 packs per year increased by 2.343 (P〈0.01), 2.309 (P〈0.05), 4.011 (P〈0.05) and 3.013 fold (P〈0.05), respectively. Conclusion It may has a certain effect on risk of pancreatic cancer for the interaction of Lys→Gln at XPD rsl3181with smoking.
