摘  要:目的探讨乳腺癌组织中局部黏着斑激酶(FAK)的表达与预后、分子分型和其他临床病理相关因素的关系。方法通过免疫组织化学法检测乳腺癌组织中FAK、雌激素受体(ER)、孕激素受体(PR)、人类表皮生长因子受体-2(Her-2)、细胞核增殖抗原(Ki-67)等的表达水平。分析310例乳腺浸润性导管癌的组织芯片中FAK表达与乳腺癌生存、分子分型以及其他临床病理特征的关系。结果生存函数(Kaplan-Meier analysis)显示FAK阳性表达与乳腺癌患者的生存呈负相关(Х^2=9.304,P〈0.01),单因素和多因素回归分析结果示FAK表达是乳腺癌不良预后的独立预后因素[P〈0.01;相对风险值(HR)=2.44;95%可信区间(CI):1.385—4.281]。FAK表达与诺丁汉组织学分级(NHG)、Her-2、Ki-67表达水平明显相关(Х^2=18.153,P〈0.01)。而FAK表达与年龄、肿瘤大小、淋巴结转移、ER、PR无关(P〉0.05)。FAK与分子亚型高度相关(Х^2=36.508,P〈0.01),且在Luminal型(Luminal A型+Luminal B型)乳腺癌以及Her-2过表达型乳腺癌中,FAK与预后相关(Х^2=3.956,P〈0.05),但在三阴性乳腺癌中FAK与生存率不相关(P〉0.05)。在淋巴结阳性的患者中,FAK与总体生存相关(P〈0.05),而在淋巴结阴性组中,FAK与总体生存无相关(P〉0.05)。结论FAK可作为乳腺癌的独立预后因子,并成为乳腺癌的潜在治疗靶点。Objective Study the correlation between focal adhesion kinase (FAK) expression and breast cancer prognosis along with other clinical and pathological parameters. Methods Analysis the expression of FAK, estrogen receptor ( ER), progesterone receptor ( PR), human epidermal - growth - factor receptor2 (Her -2), Ki -67 in 310 invasive ductal breast cancer patients purchased from the National Engineering Center for Bio Chips in Shanghai, via immunohistochemistry, these tissue samples were contained in the tissue array. All calculations were performed using SPSS Statistics version 22 software (IBM). Results Kaplan - Meier analysis demonstrated a significant correlation between FAK expression (positive vs. negative) and overall survival of breast cancer patients (Х^2 = 9. 304, P 〈 0. 01 ). Univariate and multi- variate Cox regression analysis showed that positive FAK expression was an independent indicator of poor prognosis of breast cancer patients [P 〈0. 01 ; hazard ratio (HR), 2.44; 95% confidence interval (CI) 1. 385-4. 281 ]. FAK staining intensity is significantly correlated with NHG (Nottingham histologic grading), Her -2 status, Ki -67 (Х^2 =3. 956, P〈0. 05). FAK expression was not significantly associated with age, tumor size, lymph node metastasis (P 〉 0. 05 ), nor the ER or PR status ( P 〉 0. 05, respectively). FAK is also associated with breast cancer molecular subtypes (Х^2 = 36. 508, P 〈 0. 01 ). There is also a significant correlation between FAK expression and overall survival of Luminal subtypes and Her - 2 overexpress subtype 0t,2 = 3. 956, P 〈 0.05 ), while there is no significant correlation between FAK expression and basil - like subtype. FAK is also significantly correlated with overall survival of breast cancer patients in nodular positive group (P 〈 O. 05 ), while this correlation cannot be observed in nodular negative group (P 〉 0. 05).
