摘 要:目的探讨磁共振敏感加权成像(SWI)技术对于急性缺血性脑卒中患者脑内微出血的临床诊断价值。方法对2012年6月~2015年1月曾在我院就诊并确诊为急性缺血性脑卒中的198例患者,依次行T1WI、T2WI、T2FLAIR、DWI以及SWI序列扫描。分析SWI序列和常规序列扫描在显现脑内微出血(CMB)数目以及检出敏感性方面的差异。结果 198例急性缺血性脑卒中患者中共有32例存在脑内微出血,SWI序列共检出了CMB灶207个;T1WI共检出CMB灶19个,检出敏感性为9.2%;T2WI共检出CMB灶36个,敏感性为17.4%;T2FLAIR序列共检出CMB49灶个,敏感性为23.7%;DWI序列检出CMB灶122个,敏感性为58.9%。SWI序列对CMB的检出率最高。结论 SWI序列对CMB的检出敏感性明显高于常规序列,SWI技术能明显提高CMB病灶的检出数目,对急性缺血性脑卒中的治疗与预后评价具有重要的临床参考价值。Objective To explore the susceptibility weighted imaging(SWI) in the clinical of micro bleeding diagnostic for patients with acute ischemic stroke in the brain. Methods From June2013 to August 2014, 198 patients with acute ischemic stroke with cerebral infarction underwent T1 WI, T2 WI, T2 FLAIR, DWI and the susceptibility weighted imaging(SWI). The quantity and sensitivity in detecting cerebral microbleed(CMB) was compared between the conventional MR images and SWI images. Results In 198 cases of acute ischemic stroke patients a total of 32 cases of brain hemorrhage, SWI sequence detected a CMB focal 207;T1WI, check out the CMB focal 19,detection sensitivity was 9.2%;Detection CMB36 T2 WI with detection sensitivity was 17.4%;Detection CMB49 T2 FLAIR sequence with detection sensitivity was 23.7%;DWI sequence can check out the CMB 122, detection sensitivity was 58.9%.SWI sequence of CMB has the highest detection rate. Conclusions SWI sequence on the detection sensitivity of CMB was obviously higher than that of conventional sequence, SWI technique can obviously increase the number of CMB lesions detection, evaluation for the treatment of acute ischemic stroke and prognosis have important clinical reference value.
