摘 要:目的 应用静息态低频振幅算法探讨原发性失眠(PI)患者在不同频段(Slow-4:〉0.027~0.073 Hz,Slow-5:0.01~0.027Hz)大脑自发活动的变化。方法 44例PI患者(PI组)及44名年龄、性别匹配的正常睡眠志愿者(对照组)均接受fMRI扫描,应用静息态fMRI数据处理助手(DPARSFA)进行数据预处理,SPM8软件包进行全因素方差分析PI组和对照组及不同频段脑功能活动特点。之后,对PI组和对照组采用两样本t检验分别观察两个频段ALFF值差异,同时分析ALFF值差异脑区与临床量表的相关性。结果 不同频段间一些脑区的ALFF值存在差异,包括双侧尾状核头、双侧岛叶、双侧颞上回、双侧内侧额叶、双侧眶额皮质、双侧颞下回(P〈0.01,AlphaSim校正)。与对照组相比,PI组双侧小脑后叶、左侧额上回、内侧额叶ALFF减低,而左侧颞中回、右侧颞上回、双侧后扣带回、双侧楔前叶ALFF增高(P〈0.01,AlphaSim校正)。此外,Slow-5频段更容易发现PI患者异常的大脑自发活动。结论 PI患者存在多个脑区神经元的自发活动异常且具有频率相关性。Objective To investigate the features of resting-state brain activity in amplitude of low-frequency fluctuation (ALFF) within different band of fluctuation (Slow-4:〉0. 027-0. 073 Hz; Slow-5 : 0.01-0. 027 Hz) in primary insomnia (PI group) patients. Methods Totally 44 PI patients and 44 age-, gender-, and education-matched good sleepers (GS group) were recruited and underwent R-fMRI scans. The data processing was performed using DPARSFA, measures ANOVA with condition (PI, GS) and frequency band (Slow-4, Slow-5) were performed using SPM8 to analyze the fea- tures of intrinsic brain activity, post-hoc analysis were used to observe the ALFF value between two groups in frequency bands of Slow-4 and Slow-5, and the correlations between significant ALFF values and clinical markers. Results There showed a significant ALFF value between the two bands in some regions including bilateral caudate head, bilateral insula, bilateral inferior temporal gyrus, bilateral medial frontal gyrus, bilateral orbitofrontal cortex, bilateral superior temporal gyrus (P〈0.05, AlphaSim corrected). Compared with GS group, PI group showed decreasing ALFF in the bilateral cerebellum posterior lobe,left superior frontal gyrus, medial frontal gyrus; and increasing ALFF in the left middle temporal gyrus, right superior temporal gyrus, bilateral posterior cingulate, bilateral precuneus (P〈0.05, AlphaSim corrected). Furthermore, the Slow-5 frequency band were more likely to find abnormal spontaneous brain activity in PI group. Conclusion PI patients has abnormal neurons intrinsic brain activity in many brain regions and Slow-5 band is more specific for PI.
