膀胱癌组织中Bcl-2、Bax蛋白含量检测及其与术后复发的关系研究

摘　　要：目的：研究膀胱癌组织中Bcl-2、Bax蛋白含量及其与术后复发的关系。方法：选择2008年5月～2010年2月期间在解放军总医院第一附属医院确诊并接受经尿道电切术的200例膀胱癌患者进行研究，收集膀胱癌组织和癌旁正常组织，检测Bcl-2、Bax蛋白含量，随访膀胱癌的3年复发率和5年复发率。结果：膀胱癌组织中Bcl-2的蛋白含量高于癌旁正常组织、Bax的蛋白含量低于癌旁正常组织（均Pd0．05）；不同分期和分化程度的肿瘤组织中，Bcl-2、Bax的蛋白含量有差异（均P〈0．05）；Bcl-2阳性表达膀胱癌患者的3年复发率和5年复发率均高于阴性表达患者，Bax阳性表达膀胱癌患者的3年复发率和5年复发率均低于阴性表达患者（均P〈0．05）；Bcl-2阳性表达是术后3年复发和5年复发的危险因素，OR值分别为3．283、2．894，95％C1分别为2．194—4．029、1．942—3．586（均P＜0．05）；Bax蛋白阳性表达是术后3年复发和5年复发的保护因素，OR值分别为0．138、0．171，95％C1分别为0．082—0．219、0．114—0．282（均P〈0．05）。结论：膀胱癌组织中Bcl-2的蛋白含量增多、Bax的蛋白含量减少；Bcl一2阳性表达是术后复发的危险因素、Bax蛋白阳性表达是术后复发的保护因素。Objective： To study the Bcl-2 and Bax protein contents in bladder cancer tissue and their relationship with postoperative recurrence. Methods： A total of 200 bladder cancer patients underwent transurethral resection in our hospital from May 2008 to February 2010 were enrolled for study, bladder cancer tissue and adjacent normal tissue were collected, Bcl- 2 and Bax protein contents were detected, and follow up was conducted to investigate the three-year and five-year recurrence rates. Results Bcl 2 protein content in bladder cancer tissue was higher than that in adjacent normal tissue, but Bax protein content was lower than that in adjacent normal tissue （P all 〈0.05）. Bcl-2 and Bax protein contents in tumor tissue with dif ferent stages and degrees of differentiation were significant different （P all〈0.05）. Three-year and five-year recurrence rates of patients with Bcl-2 positive expression were higher than those of patients with negative expression, but three year and five year recurrence rates of patients with Bax positive expression were lower than those of patients with negative expression （P all d0.05）. Positive expression of Bcl-2 was a risk factor for postoperative 3-year recurrence and 5-year recurrence with OR values of 3. 283 and 2. 894 and 95%CI of 2. 194-4. 029 and 1. 942-3. 586, （P all 〈0.05）. On the other hand, positive expression of Bax was a protective factor of postoperative 3-year recurrence and 5-year recurrence with OR values of 0. 138 and 0. 171,95 % CI of 0. 082-0. 219 and 0. 114-0. 282 （P all 〈0.05）. Gonclusions： Bcl-2 protein content increases and Bax protein content decreases in bladder cancer tissue indicating positive expression of Bcl-2 is a risk factor of postoperative recurrence but positive expression of Bax is a protective factor of postoperative recurrence.
