病毒性肝炎合并血小板减少症134例的临床分析

摘　　要：目的对134例病毒性肝炎合并血小板减少症进行临床分析。方法入选我院2014年7月至2015年8月病毒性肝炎患者134例为观察组,入选同期体检健康者134例为对照组,比较两组γ-转肽酶(γ-GT)、碱性磷酸酶(ALP)、谷草转氨酶(AST)、谷丙转氨酶(ALT)等临床指标;比较两组的血小板减少水平、血小板计数(PLT)等指标;应用流式细胞术检测血小板相关免疫球蛋白(PAlg)及其类别PAlg G、PAlg A、PAlg M水平。应用酶联免疫吸附法检测血清促血小板生成素(TPO)水平。结果观察组的γ-转肽酶(γ-GT)、碱性磷酸酶(ALP)、谷草转氨酶(AST)、谷丙转氨酶(ALT)等临床指标显著高于对照组,差异有统计学意义(P<0.05);观察组PLT减少水平、PLT等临床指标均显著低于对照组,差异有统计学意义(P<0.05);观察组的PAlg、PAlg G指标显著高于对照组,TPO显著低于对照组,差异有统计学意义(P<0.05),而两组的PAlg M、PAlg A指标差异无统计学意义(P>0.05)。结论病毒性肝炎合并血小板减少症与肝脏受损合成减少密切相关,同时介导PAIg的免疫机制。Objective To analyze 134 cases of viral hepatitis complicated with thrombocytopenia. Methods 134 cases of viral hepatitis treated in our hospital from July 2014 to August 2015 were selected as observation group, and 134 healthy check-ups were selected as control group. The levels of γ-GT, ALP, AST and ALT of two groups was compared; through enzyme linked immunosorbent assay, the level of serum thrombopoietin（TPO） were detected. Flow cytometry was used to detect the expressions of PAIg, PAIg G, PAIg A and PAIg M.Results The levels of γ-GT, ALP, AST and ALT of observation group were significantly higher than those of control group（P〈0.05）. The reduced level of PLT, and PLT level of observation group were significantly lower than those of control group（P〈0.05）. The levels of PAlg and PAlg G of observation group were significantly higher than those of control group, and the TPO level was significantly lower than that of control group（P〈0.05）. The levels of PAlg M and PAlg A between two groups were not significantly different（P〉0.05）. Conclusions The viral hepatitis complicated with thrombocytopenia is closely related with hepatic damage and mediates PAIg immune mechanism.
