肿瘤患者上肢静脉烟雾影与血流参数、凝血指标关系的研究

摘　　要：目的:探讨肿瘤患者上肢静脉烟雾影与超声血流参数、凝血指标、血栓前状态的关系。方法:选取55例经上肢静脉置入PICC管化疗的恶性肿瘤患者,根据上肢静脉有无烟雾影分为烟雾影组25例及无烟雾影组30例,回顾性分析两组患者的超声血流参数及凝血指标。结果:两组锁骨下静脉、腋静脉、肱静脉血管内径比较,二者无统计学差异(P>0.05)。两组锁骨下静脉、腋静脉、肱静脉血流Vmax比较,烟雾影组明显低于无烟雾影组,二者差异有统计学意义(P<0.05)。两组PT、APTT、FIB、D-D数值比较,烟雾影组PT、APTT值均小于无烟雾影组,FIB、D-D值均大于无烟雾影组,两组PT、D-D、FIB值有统计学差异(P<0.01),APTT值无统计学差异(P>0.05)。结论:肿瘤患者PICC术后上肢静脉烟雾影的出现,对血栓前状态具有预警价值。Objective： To explore the relationship between smoke-like echo in veins of upper limbs and parameters of blood flow, coagulation indexes, and pre-thrombosis state in patients with cancer. Methods： 55 patients with malignant tumor undergoing chemotherapy via peripherally inserted central catheter （PICC） were enrolled. The patients were classified to two groups according to presence of smokelike echo in veins of upper limbs, and there were 25 patients in group with smoke-like echo in veins of upper limbs, whereas 30 patients in group without smoke-like echo in veins of upper limbs. Blood flow parameters and coagulation indexes in both groups were analyzed retrospectively. Results： There were no statistical differences in diameters of subclavicular, axillary and brachial veins between the two groups （P〉0.05）. Vmax of subclavicular, axillary and brachial veins were significantly lower in smoke-like echo compared to the parameters in group without smoke-like echo （P〈0.05）. There were lower values of PT, APTT and higher values of fibrinogen and D-Dimer in smoke-like echo compared to the parameters in group without smoke-like echo. Of these parameters, there were significant statistical differences in val- ues of PT, D-Dimer and fibrinogen in smoke-like echo compared to without smoke-like echo（P〈0.01）, but no statistical difference in APTT value in the two groups （P〉0.05）. Conclusion： smoke-like echo in veins of upper limbs is of predictive value for pre-thrombosis state in patients with malignant tumor after they received PICC catheterization.
