摘  要:目的研究DNA结合分化抑制蛋白1(Id-1)在结直肠癌细胞增殖、分化过程中的重要作用。方法选取手术切除腺瘤伴不典型增生30例(中低度16例、重度14例)、腺癌50例、邻近正常黏膜50例,通过免疫组化、荧光原位杂交方法检测其Id.1的表达情况,并研究其与结直肠癌相关临床病理因素之间的关系。结果Id-1蛋白在结直肠正常黏膜、中低度不典型增生腺瘤、重度不典型增生腺瘤、腺癌中的阳性表达率分别为24.00%、31.25%、57.14%、72.00%;Id-1mRNA在结直肠正常黏膜、中低度不典型增生腺瘤、重度不典型增生腺瘤、腺癌中的阳性表达率分别为20.00%、25.00%、57.14%、76.00%;Id-1蛋白及Id-1 mRNA在正常黏膜组织、腺瘤伴中低度不典型增生、腺瘤伴重度不典型增生、腺癌中的阳性表达逐步增强(P〈0.01);Kappa一致性检验显示,在正常黏膜、腺瘤、腺癌组织中吻合系数分别为k:0.791、0.729、0.905;从TNMⅠ-Ⅳ期Id-1表达也逐渐增强,差异有统计学意义(P〈0.01);Id-1表达与肿瘤浆膜浸润、CEA(+)、淋巴结转移、肝转移及脉管浸润等密切相关(P〈0.01),而与肿瘤大小及分化程度无关。结论Id-1异常表达在肿瘤癌变过程中发挥了重要作用,有望成为新的肿瘤监测指标,为临床诊治及预后判断指明新的方向。Objective To study the expression of Id-1 in human colorectal adenocarcinoma tissues and explore its correlation with the initiation and progression of colorectal caneer. Methods The expression of Id-1 in 50 specimens of normal colorectal tissues,50 specimens of eoloreetal adenomaearcinoma tissues,and 30 colorectal adenoma tissues (moderate-low dysplasia of 16 eases and high dysplasia of 14 eases) were detected using immunohistoehemistry technique and in situ hybridization. The correlation between its expression and the elinieopathologie features was analyzed. Results The positive expression rates of Id-1 protein in colorectal normal tissues, colorectal adenoma tissues with moderate-low dysplasia, coloreetal adenoma tissues with high dysplasia, and eoloreetal adenocareinoma tissues,were 24.00%, 31.25% ,57. 14% , and 72.00%. The positive rates of Id-1 mRNA in colorectal normahissues,colorectal adenocarcinoma tissues with moderate-low dysplasia, colorectal adenocarcinoma tissues with high dysplasia, and colorectal adenocarcinoma tissues ,were 20. 00% ,25.00% ,57. 14%, and 76. 00%. The positive expression rates of Id-1 protein and Id-1 mRNA in colorectal normal tissues, colorectal adenocarcinoma tissues with moderate-low dysplasia, colorectal adenocarcinoma tissues with high dysplasia, and colorectal adenocarcinoma tissues gradually increased and the differences had obvious statistical significance (P 〈 0. 01 ). The coincidence coefficient in normal colorectal tissues, adenoma tissues, and colorectal adenocarcinoma tissues were 0. 791,0. 729, and 0. 905 by Kappa testing. The positive expression level from TNMI to TNMIV stage in colorectal adenocarcinoma tissues increased gradually, and there was statistical significance (P 〈 0.01 ). Expression of Id-1 was correlated with the depth of tumor invasion, CEA ( + ), lymph node metastasis, vessel invasion, and liver metastasis (P 〈 0. 01 ), but not with the patient' s tumor size, and differentiation degrees.
