阿尔茨海默病与胰岛素及胰岛素抵抗指数的关系研究

摘　　要：目的探讨阿尔茨海默病(AD)与胰岛素及胰岛素抵抗(HOMA2-IR)指数的关系。方法 95例AD患者作为病例组,其中轻度34例,中度37例,重度24例,80例健康体检者作为对照组,检测血糖、血脂、空腹胰岛素(FINS)、2h胰岛素(2h INS)、胰岛素抵抗(HOMA2-IR)指数等指标,并分析各指标与患者认知功能的相关性。结果病例组与对照组间、不同严重程度AD组间的空腹血糖(FBG)、餐后2h血糖(2h PBG)、2h INS、血脂水平比较,差异均无统计学意义(P>0.05);病例组FINS、HOMA2-IR指数高于对照组(P<0.05),简明精神状态量表(MMSE)评分低于对照组(P<0.05);轻度组FINS、HOMA2-IR指数低于重度组(P<0.05),MMSE评分高于中度、重度组(P<0.05);FINS、HOMA2-IR指数与MMSE评分呈负相关(P<0.05)。结论 AD患者FINS、HOMA2-IR指数明显升高,且与MMSE评分呈负相关,可作为疾病早期诊断的重要观察指标。Objective To investigate the relationship of Alzheimer＇s disease （AD） with insulin and homeostasis model assess- ment 2-insulin resistance（HOMA2-IR） index. Methods A total of 95 patients with AD were enrolled as case group, including 34 with mild AD,37 with moderate AD and 24 with severe AD. A total of 80 healthy individuals were enrolled as control group. Blood glucose, blood lipids ,fasting insulin（FINS） ,2-honr postprandial insulin（2hINS）, HOMA2-IR index were detected in both groups. The correlation between the indices and the cognitive function of patients was analyzed. Results There were no significant differences in the levels of fasting blood glucose（ FBG）, 2-honr postprandial blood glucose （2hPBG） ,2hINS or blood lipids between case group and control group, or among different severity AD groups （P 〉 0.05）. FINS and HOMA2-IR index in the case group were significantly higher than those in the control group（ P 〈 0.05 ） ,while Mini-Mental State Examination （MMSE） score in the case group was significantly lower than that in the control group（P 〈0.05 ）. FINS and HOMA2-IR index in the mild AD group were significantly lower than those in the severe AD group （ P 〈 0.05 ）, and MMSE score in the mild AD group was significantly higher than that in the moderate or severe AD group （ P 〈 0.05 ）. MMSE score negatively correlated with FINS or HOMA2-IR index （P 〈 0.05 ）. Conclusion FINS and HOMA2-IR index significantly increase in patients with AD and negatively correlate with MMSE score,which can be used as an important index for the early diagnosis of the diseases.
