腹腔镜辅助直肠癌根治术的近期疗效

摘　　要：目的比较腹腔镜辅助直肠癌根治术和开腹直肠癌根治术的近期疗效。方法分析直肠癌根治术患者85例的临床资料。其中,腹腔镜辅助直肠癌根治术45例(腔镜组),开腹直肠癌根治术40例(开腹组),比较两组的手术时间、术中出血量、淋巴结清扫数目、术后肠功能恢复时间和术后住院时间。结果腔镜组的手术时间长于开腹组[(165.82±23.65)min vs.(106.35±18.31)min](P<0.01);但是腔镜组较开腹组术中出血量减少[(152.97±96.56)ml vs.(302.10±141.92)ml](P<0.05),术后肠功能恢复时间[(3.40±1.34)d vs.(4.93±1.21)d]及术后住院时间[(10.93±4.02)d vs.(13.60±5.56)d]缩短(P<0.05)。两组术中淋巴结清扫数目和术后并发症发生率相仿(P>0.05)。结论与传统开腹手术比较,腹腔镜辅助直肠癌根治术具有术中出血少、术后肠功能恢复快和住院时间短等优势。Objective To compare the short-term clinical efficacy of laparoscope-assisted radical resection for rectal cancer with conventional open surgery for rectal cancer.Methods Clinical data of 85 patients with rectal cancer underwent radical resection were analyzed,of whom 45 cases received laparoscope-assisted radical resection（group A）and 40 cases underwent conventional open radical resection（group B）.The short-term outcomes were compared between two groups.Results The operation time was longer in group A than that in group B[（165.82±23.65）min vs.（106.35±18.31）min]（P〈0.01）.But blood loss during surgery was less in group A than that in group B[（152.97±96.56）ml vs.（302.10±141.92）ml]（P〈0.05）.The recovery of gastrointestinal function and hospital stay after operation were shorter in group A than those in group B[（3.40±1.34）d vs.（4.93±1.21）d and（10.93±4.02）d vs.（13.60±5.56）]（P〈0.05）.The number of lymph node resected and the incidence of complications in group A were similar to those in group B（P〉0.05）.Conclusion Compared to conventional open surgery,laparoscope-assisted radical resection for rectal cancer has the advantages of less blood loss during surgery,earlier recovery of gastrointestinal function and shorter hospital stay after operation.
