唑来膦酸联合放疗治疗恶性肿瘤骨转移患者的临床观察

摘　　要：目的观察唑来膦酸联合放疗治疗恶性肿瘤骨转移患者的疗效.方法：将70 例恶性肿瘤骨转移患者分为唑来膦酸联合放疗组和单纯放疗组,每组各35 例.唑来膦酸联合放疗组患者于第l 天应用唑来膦酸4 mg 静滴,每4 周重复,不少于4 次,第2 天开始行局部放疗,常规分割DT 2 Gy/ 次,5 次/ 周,总量40 -50 Gy;大分割DT 3 Gy/ 次,5 次/ 周,总量30 Gy;单纯放疗组患者行单纯局部放疗,观察两组患者的临床效果及不良反应情况.结果：唑来膦酸联合放疗组患者的疼痛缓解有效率为88. 57%,单纯放疗组为74. 29%,唑来膦酸联合放疗组患者的有效率高于单纯放疗组（P〈0. 05）;唑来膦酸联合放疗组患者的活动能力改善总有效率为85. 72%,单纯放疗组总有效率为68. 57%,唑来膦酸联合放疗组患者高于单纯放疗组（P〈0. 05）;两组患者均未出现严重放疗或药物毒副反应.结论：唑来膦酸联合放疗治疗恶性肿瘤骨转移患者的疗效优于单纯放疗.Objective: To observe efficacy of Zoledronic acid combined with radiation therapy in treatment of metastatic bone cancer. Methods: Seventy patients with bone metastases were equally divided into two groups,with 35 cases in each group. In the combined therapy group,the patients were treated with 4 mg Zoledronic acid intravenously on day 1,once every 4 weeks,for at least four times,and then given local radiation of DT 2 Gy / time,5 times / week,40 ~ 50 Gy with conventional fractionation and DT 3 Gy /time,5 times / week,30 Gy with larger fractionation. In the radiotherapy group,the patients underwent radiotherapy alone. The clinical efficacy and adverse reactions in the two groups were observed. Results: The effective rate of pain relief in the combined therapy group was higher than that in the radiotherapy group( 88. 57% vs. 74. 29%,P < 0. 05). The total activity improvement in the combined therapy group was also higher than that in the radiotherapy group( 85. 72% vs. 68. 57%,P < 0. 05). No cases in two groups appeared severe radiotherapy or drug toxic reactions. Conclusions: Zoledronic acid combined with radiation therapy has a clear effect in the treatment of bone metastases than single radiation therapy.
