合理情绪行为疗法对晚期女性乳腺癌患者心理干预的作用

摘　　要：目的晚期女性乳腺癌患者往往面临严重的心理问题,本文将探讨合理情绪疗法在患者心理干预中的作用,以期提高其心理压力应对能力及生活质量。方法对126例晚期女性乳腺癌患者按随机数字表法分为实验组和对照组,对照组给予肿瘤科常规干预,实验组另给予合理情绪疗法作心理干预,分别采用症状自评量表、艾森克个性问卷和世界卫生组织生活质量量表评价心理干预前后患者的心理状况、人格和生活质量。结果合理情绪行为疗法明显改善了患者在躯体化、强迫、人际敏感、抑郁、焦虑和恐怖等6个方面的心理状态,SCL-90量表中实验组阳性项目数较干预前减少(3.2±1.4)个,而对照组仅减少了(2.0±0.9)个,实验组总均分改善了1.7±0.6,而对照组仅改善了0.7±0.4,同时在一定程度上改善患偏于内向、情绪不稳和精神质等不良个性,并且提高了患者在心理、独立性、社会关系和精神支持等5个方面的生活质量,WHOQOL-100量表实验组总分为43.1±7.2,而对照组仅为27.2±8.3。结论合理情绪不仅可以改善晚期女性乳腺癌患者的心理状态,提高其对心理压力的应对能力,同时提高了患者的生活质量,值得推广应用。Objective The female patients with advanced mammary cancer commonly suffer from serious psychological problem. The aim of this study is to explore the function of rational emotive behavior therapy in psychological intervention in order to improve their psychological stress response capacity and quality of life. Methods A total of 126 female pa- tients with advanced mammary cancer in our department were divided into experiment group and control group, the normal nursing was given to all patients, and mental nursing of rational emotive behavior was additional given to the patients of ex- periment group. Symptom checklist 90, Eysenck Personality Questionnaire and World Health Organization Quality of Life- 100 were adopted to evaluate the mental condition,personality and living quality. Results The rational emotive behavior therapy had improved the mental state of patients such as somatization, force, interpersonal sensitivity, depression, anxiety and terror etc. , the number of positive factor in SCL-90 reduced 3.2 ± 1.4 in the experimental group and 2.0 ± 0.9 in the control group, the improvement of total average was 1.7 ± 0.6 in the experimental group and 0.7 ± 0.4 in the control group. The rational emotive behavior therapy had also improved the introverted, emotional instability and psychotieism, as well as the quality of life such as psychological, independence, social relation and mental support. The total score of WHO- QOL-100 was 43.1 ± 7.2 in the experimental group and 27.2 ± 8.3 in the control group. Conclusion The reasonable e- motion therapy could obviously improve the mental state and quality of life in patients with advanced breast cancer, and is worthy of popularization and application in psychological therapy.
