气/液造影在超声引导下乳腺肿瘤微创旋切术中的应用

摘　　要：目的探讨气/液造影技术在超声引导下乳腺肿瘤微创旋切术中的临床应用价值。方法回顾性分析我院2013年1月~2015年1月超声引导下乳腺肿瘤微创旋切术620例1436个肿瘤的资料,按不同医生分组,其中气/液造影组308例,普通显像组312例,比较2组手术时间、术中出血量、住院时间、美容满意度评分,以及血肿、术后1个月仍感疼痛、肿瘤复发/残留的发生率。结果 2组血肿、术后1个月仍感疼痛、肿瘤复发/残留发生率差异无统计学意义（P〉0.05）。气/液造影组的手术时间、术中出血量、术后住院时间和术后6个月美容满意度评分均优于普通显像组[（20.8±4.2）min vs.（35.9±3.1）min,t=-50.881,P=0.000;（5.0±0.4）ml vs.（10.2±0.8）ml,t=-102.559,P=0.000;（48.6±0.5）h vs.（72.3±0.2）h,t=-761.176,P=0.000;（4.8±0.1）分vs.（4.2±0.2）分,t=46.460,P=0.000]。结论与普通显像相比,气/液造影技术应用于超声引导下乳腺肿瘤微创旋切术具有手术时间短、术中出血量少、恢复快、美容效果更好等优势,且支持黑白超声、一人操作手术模式。Objective To investigate the clinical value of gas/liquid contrast radiography in ultrasound-guided vacuumassisted minimally invasive breast biopsy. Methods We retrospectively analyzed documents of 1436 breast lesions from 620 cases,who were treated with ultrasound-guided vacuum-assisted minimally invasive breast biopsy in our hospital from January 2013 to January2015. Among them,308 cases were given gas / liquid contrast radiography and 312 cases were given traditional imaging. The operative time,blood loss,hospital stay,cosmetic scores,as well as incidence of hematoma,postoperative pain after one month,recurrent /residual tumor were compared between the two groups. Results No significant differences between the two groups were found in incidence of hematoma,postoperative pain after one month,and recurrent / residual tumor（ P〉0. 05）. The operative time,blood loss,hospital stay,and cosmetic scores 6 months after the surgery in the gas / liquid radiography group were significantly better than those in the traditional imaging group [（ 20. 8 ± 4. 2） min vs.（ 35. 9 ± 3. 1） min,t =- 50. 881,P = 0. 000;（ 5. 0 ± 0. 4） ml vs.（ 10. 2 ±0. 8） ml,t =- 102. 559,P = 0. 000;（ 48. 6 ± 0. 5） h vs.（ 72. 3 ± 0. 2） h,t =- 761. 176,P = 0. 000;（ 4. 8 ± 0. 1） points vs.（ 4. 2 ± 0. 2） points,t = 46. 460,P = 0. 000]. Conclusion As compared to traditional imaging,the gas/liquid contrast radiography has shorter operative time,less blood loss,faster recovery,better cosmetic results and other advantages,feasible to black-and-white ultrasound device and single operator mode.
