摘 要:背景角膜生物力学特性是角膜屈光手术安全评估的重要指标。Corvis是测量角膜生物力学特性的新型设备。目的观察角膜厚度、Corvis眼压测量值与角膜生物力学特性的相关性。方法采用前瞻性观察研究,纳入2013年8月至2014年2月在第三军医大学大坪医院野战外科研究所眼科拟行准分子角膜屈光术患者75例150眼,根据患者角膜厚度分为3个组:低角膜厚度组:角膜厚度501~530μm;中角膜厚度组:角膜厚度531~560μm;高角膜厚度组:角膜厚度561~590μm;每组25例50眼。采用单因素方差分析比较3个组间眼压、角膜厚度和角膜变形幅度(DA)的差异性。采用多元线性回归分析各组内DA与角膜厚度、眼压的相关性。结果低角膜厚度组与中角膜厚度组DA显著高于高角膜厚度组,差异具有统计学意义(P〈0.05)。3个组间眼压总体比较差异具有统计学意义(F=9.98,P〈0.05)。DA与眼压呈高度负相关(r=-0.84,P〈0.01),与角膜厚度呈负相关(r=-0.33,P〈0.01)。线性回归方程:DA=1.69—0.04×眼压(F=366.19,P〈0.05;t=-19.14,P〈0.01)。结论角膜屈光手术安全性的评估不能单纯以角膜厚度代替角膜生物力学特性,需参考眼压等因素的影响。Background Corneal biomechanical properties is important in the safety assessment of corneal refractive surgery. Corvis is a new device for measuring corneal biomeehanics properties. Objective This study was to observe the correlation among corneal thickness, Corvis intraocular pressure and corneal biomechanieal properties with Corvis. Methods A prospective observational study was performed. One hundred and fifty eyes of 75 patients with corneal thickness from 501μm to 590μm were divided into three groups according to the corneal thickness:low corneal thickness group (corneal thickness range from 501μm to 530μm) ,middle corneal thickness group (corneal thickness range from 531μm to 560μm) ,and high corneal thickness group (corneal thickness range from 561μm to 590μm) ;and 50 eyes of 25 patients for each group. The difference of intraocular pressure, corneal thickness and deformation amplitude (DA) among the three groups were analyzed by one-way ANOVA and the correlation among the groups were analyzed by liner regression. Results The DA in the low corneal thickness group and middle corneal thickness group were significantly higher than that in the high corneal thickness group (P〈 0. 05). The intraocular pressure was statistically different among the 3 groups (F = 9.98,P〈0.05 ). DA was negatively correlated with intraocular pressure and corneal thickness ( r = -0. 84, -0. 33 ; both at P〈0. 01 ) , with the linear regression DA = 1.69-0.04×IOP ( F = 366. 19, t = - 19.14,P〈0.01 ). Conclusions Corneal thickness cannot simply represent the corneal biomechanical properties in the safety assessment of corneal refractive surgery,lOP should be considered.
