摘  要:目的:应用超声射频量化技术评价脑梗死患者颈动脉内膜-中层厚度(IMT)与动脉僵硬度的变化。方法:选取脑梗死患者54例为梗死组,同期健康志愿者50例为对照组。应用超声射频信号血管内中膜分析(QIMT)技术和超声射频信号动脉僵硬度分析(QAS)技术对2组右颈总动脉进行检查,获其颈总动脉IMT、血管僵硬度参数(β)、动脉管壁的扩张性系数(DC)和顺应性系数(CC)。结果:梗死组和对照组相比颈总动脉IMT差异无统计学意义(P>0.05);CC、DC值低于对照组,差异有统计学意义(P<0.05);β值高于对照组,有显著性差异(P<0.01)。2组的收缩压(SBP)和低密度脂蛋白(LDL)差异有统计学意义(P<0.05)。相关分析显示梗死组的β值与SBP、LDL呈正相关(r=0.75,P=0.012;r=0.64,P=0.016)。结论:脑梗死患者颈动脉管壁僵硬度的增加早于内中膜增厚的改变,超声射频信号技术可敏感反映颈动脉结构和功能的改变。Objective:To assess the changes of carotid arterial quantitative intima-media thickness (IMT) and quantitative arterial stiffness (QAS) in patients with cerebral infarction by ultrasound radio-frequency technique. Methods: Fifty-four patients with cerebral infarction (infarction group) and 50 healthy volunteers (control group) were enrolled. The structural and functional changes in the carotid artery wall were investigated by quality IMT (QIMT) and QAS techniques. The IMT, compliance coefficient (CC), distensibility coefficient (DC) and stiffness index ([3) of the right carotid artery were measured in the 2 groups. Results: There was no significant differences in the IMT between the 2 groups (P〉0.05). The DC and CC in the infarction group were significantly lower than those in the control group (P〈0.05), while β value was significantly higher than that in the case group (P〈0.01). The systolic blood pressure (SBP) and low density lipoprotein (LDL) were significantly different between the 2 groups (P〈0.05). SBP and LDL were positively correlated with β value (r=0.75, P〈0.012; r=0.64, P=0.016). Con- clusion: Patients with cerebral infarction show increased arterial stiffness which is prior to changes in carotid inti- ma-media thickness. The ultrasound radio-frequency technique could reliably reflect the changes of the carotid artery structure and elasticity.
