超声引导下激光消融甲状腺良性肿物的初步应用

摘　　要：目的探讨激光消融甲状腺良性肿物的疗效、并发症及原因。方法选择2012年12月至2013年3月于我院行超声引导下甲状腺肿物激光消融的患者20例共47个结节进行分析对比。术前病例均甲状腺功能正常,结节均经穿刺病理证实为良性结节。消融后1个月、3个月、6个月、12个月复查超声及甲状腺功能。结果 20例患者共47个结节观察12个月后完全消失17个(36.2%),按体积计算缩小超过50%者21个(44.7%),缩小大于25%不足50%者8个(17%),无明显变化者1个(2.1%)。在全部20例患者中,术后出现头晕、头痛者7例(35%),牙痛1例(5%),颈前区疼痛14例(70%),均于术后1周内消失。声音嘶哑6例(30%),其中5例(25%)于术后1h内恢复,1例(5%)于术后1个月恢复。全部20例患者术后检查甲状腺功能均正常。结论初步应用超声引导下激光消融治疗甲状腺良性肿瘤安全、有效,且甲状腺功能不受影响。Objective To investigate the curative effect, complications and their reasons of laser ablation on thyroid benign nodule. Methods 20 patients with 47 nodules are selected to be analyzed and compared,who were treated by la- ser ablation on thyroid nodules guided by ultrasound during Dec 2012 and Mar 2013. Their thyroids had normal func tions,and their nodules had been proved to be benign by aspiration biopsy. Thyroid nodule volume and thyroid function were evaluated 1,3,6,and 12 months after treatment. Results After 12 months,among the 47 nodules,17（36. 2%） nodules disappeared,21 nodules（44.7X） reduced their volumes over 50G ,8 nodules（17%） reduced their volumes less than 50% but over 25% ,and 1 nodule（2.1%） did not obviously change. After laser ablation,7 patients （35%） had diz ziness or headache, 1 patient （5 %） had toothache, and 14 patients （70 % ） had sore of anterior cervical, and each symp- tom disappeared one week after the operation. 6 patients （30 %） had vocal cord paresis,of whom 5 patients （25 %） had recovered in one hour,and 1 patient （5%） in one month. All these 20 patients＇ thyroids were proved to have normal functions after the operation. Conclusion Preliminary applications of laser ablation on thyroid benign nodules guided by ultrasound prove to be safe and valid,while the thyroid＇s functions will not be affected.
