摘  要:目的 明确新型血红蛋白携氧载体OC33是否具有抗猪重度失血性休克的作用。方法无特定病原(specified—pathogensfree,SPF)级小香猪,雌雄各半,8~10月龄,体重25~30kg。制备50%失血量重度失血性休克猪模型,随机分为乳酸林格式液组(LR),全血组(WB),OC33低、中、高剂量组(0.1gHb/kg、0.3gHb/kg、0.5gHb/kg)进行治疗,每组6只。测定组织氧供(DO2)、氧耗(VO2)、血流动力学、心肌力学、心输出量及心功能等指标,记录24h存活率与存活时间,进行统计学处理以明确OC33是否具有抗猪重度失血性休克的作用。结果各剂量OC33组模型动物氧供、氧耗、血流动力学、心输出量、心肌力学指标(MAP、LVSP、LVEDP、+/-dp/dtmax)、心功能(CO、CI、SI)、存活时间与存活率等指标均优于LR组,其中0.5gHb/蝇组效果接近WB组。结论OC33具有较好的抗猪重度失血性休克的效果。Objective To investigate the effect of the hemoglobin based oxygen carrier, OC33, in severe hemorrhagic shock in pig models. Methods Eighteen severe hemorrhagic shock pig models ( 9 male pigs and 9 females pigs of 8-10 years weighted from 25-30kg) were prepared from specified pathogens free (SPF) pigs by controlled blood depleting of 50% whole blood volume. And then model pigs were randomly divided into three groups, Ringer's solution team ( LR ) , whole blood group ( WB ) and OC33 group. Model pigs were treated with corresponding solutions, and indexes including tissue oxygen delivery ( DO: ) and uptake ( VO2 ), blood kinetics and myocardial kineticscardiac output and cardiac function were tested. Model pigs were sent back to observation ward after remedy and 24 hour survival time and survival rate were recorded. Statistical analysis was done to evaluate whether OC33 could be used in severe hemorrhagic shock pig remedy. Results Indexes including DO2,VO2 , blood kinetics,heart output,myocardial mechanic parameters ( MAP, LVSP, LVEDP, + / - dp/dtmax ) , heart function parameters (CO, CI,SI) ,survival rate and time of the OC33 group were better than those in the LR group. The 0. 5g Hb/kg group had the best effect among the three OC33 groups, some indexes of this group were even similar with those of the WB group. Conclusion OC33 is a good candidate in severe hemorrhagic shock remedy for pig models.
