缺血性脑卒中患者脑微出血的相关危险因素及对认知功能的影响

摘　　要：目的探讨缺血性脑卒中患者脑微出血的相关危险因素及对认知功能的影响。方法回顾性分析2009-12-2013-12在中国人民解放军第254医院和河北医科大学第二医院神经内科住院治疗的132例缺血性脑卒中患者的临床资料,分为CMB组45例,非CMB组为87例,对比分析2组的相关危险因素及对认知功能的影响。结果 2组患者一般资料单因素分析比较显示,年龄、高血压、高血脂、脑梗死、腔隙性梗死数目、脑白质改变、服用抗血栓药物史组间差异有统计学意义(P<0.05);多因素非条件的Logistic回归分析显示,其均为脑微出血的独立危险因素;2组患者认知功能比较显示,CMB组较非CMB组各项认知功能评分均较低,差异均有统计学意义(P<0.05)。结论高血压、高血脂、脑梗死、脑白质改变、服用抗血栓药物及腔隙性梗死数目可能为CMBs的危险因素,防治上述因素对于预防缺血性脑卒中患者脑微出血和认知功能障碍均有一定作用。Objective To explore the related risk factors of micro-hemorrhage and its effects on cognitive function in ischemic stroke patients.Methods The clinical data of 132 cases with ischemic stroke in our hospital from December 2009 to December 2013 were reviewed,including 45 cases forming CMB group and the remaining 87 cases regarded as non-CMB group,to analyze the related risk factors and the influence on cognitive function.Results General information tested by univariate analysis showed that the age,hypertension,hyperlipidemia,cerebral infarction,the number of lacunar infarction,white matter changes and the history of taking antithrombotic prescription drugs in two groups had statistically significant differences（P〈0.05）,all of which were certified as independent risk factors by multivariate Logistic regression analysis.The score of cognitive function in CMB group were significantly lower than that of the non-CMB group with a statistical difference（P〈0.05）.Conclusion Hypertension,hyperlipidemia,cerebral infarction,the number of lacunar infarction,white matter changes and the history of taking antithrombotic prescription drugs may be possible risk factors for micro-hemorrhage patients with ischemic stroke,we should take steps to prevent to improve cognitive function.
