穴位注射联合中医综合理疗治疗脑卒中后上肢运动功能障碍的临床观察

摘　　要：目的:观察穴位注射联合中医综合理疗治疗脑卒中后上肢运动功能障碍的临床疗效。方法:临床纳入脑卒中后上肢运动功能障碍患者90例,根据康复训练方案的不同分为治疗组与对照组。治疗组进行穴位注射联合中医综合理疗,对照组进行肌电生物反馈。结果:治疗前,两组患者FMA、MBI、IEMG评分差异均无显著性(P>0.05);治疗后,治疗组FMA、MBI评分明显高于对照组,差异均有显著性(P<0.05);治疗后,治疗组患侧肱二、三头肌最大等长收缩时IEMG评分明显高于对照组,差异有显著性(P<0.05)。结论:穴位注射联合中医综合理疗治疗对改善脑卒中后上肢运动功能障碍患者有着较好的临床效果,能够显著提高偏瘫患者上肢功能以及生活质量,值得应用。Objective：To study the clinical effect of acupoint injection combined with integrated physiotherapy of TCM in upper extremity dyskinesia after stroke. Methods：90 stroke patients with upper extremity dyskinesia were se- lected and divided into treatment group and control group according to different rehabilitation training. The treatment group adopted the acupoint injection combined with integrated physiotherapy of TCM; the control group adopted the elec- tromyographic biofeedback. Through the simplified FMA （upper extremity）, BMI as well as IEMG of biceps brachii and triceps brachii when the elbow joint reached up to the maximaI isometric contraction, the dyskinesia, self-help ability and neuromuscular function of two groups before and after treatment was observed. Results： Before treatment, the score of FMA, MBI and IEMG of two groups was not significantly different （P〈0.05）; after treatment, the score of FMA and MBI of treatment group was （34.30±15.04） and （62.31±8.43） ; the score of FMA and MBI of control group was （25. 08±12.51） and （56.64±8. 98） （P〈0. 05）; when the elbow joint reached up to the maximal isometric contraction of treatment group, the score of IEMG of biceps brachii and triceps brachii was （52.30±19.45） and （44.97±13.14） while the score of IEMG of control group was （45.23±17.13） and （41. 48±12.37） （P〈0.05）. Conclusion： The acupoint in- jection combined with integrated physiotherapy of TCM has a favorable clinical effect in the improvement of upper extrem- ity dyskinesia after stroke and can greatly enhance the upper extremity function and quality of life for hemiplegic patients. It is worthy of promotion.
