高压氧对直肠癌患者免疫功能及肿瘤生长的影响

摘　　要：目的探讨高压氧对直肠癌患者行Miles术后免疫功能及肿瘤复发情况的影响。方法选取2012年10月至2013年10月间收治的经病理诊断明确的直肠癌患者210例,根据随机数字表法分为高压氧组(n=106)和对照组(n=104)。高压氧组患者在行肠外静脉营养支持及伤口换药处理基础上于术前1天、术后第3、4、5、6、7天行高压氧治疗,对照组患者行肠外静脉营养支持及伤口换药处理,检测术前1天以及术后7天患者外周血白介素2(IL-2)、白介素4(IL-4)及干扰素γ(INF-γ)的含量,对患者进行随访,于术后6个月行正电子发射计算机断层显像/计算机断层扫描(PET/CT)检查观察肿瘤复发情况。结果高压氧组患者IL-2和INF-γ的含量较对照组明显增多(P<0.05),而高压氧组患者IL-4的含量较对照组中明显减少(P<0.05)。与治疗前相比,高压氧组患者IL-2、IL-4和INF-γ治疗后均有显著变化(P<0.05),对照组患者IL-4和INF-γ治疗后有显著变化(P<0.05)。高压氧组患者在术后半年复发率显著低于对照组(P<0.05)。结论高压氧治疗能增强直肠癌行Miles术患者术后细胞免疫功能,可有效预防直肠癌术后半年复发率,值得进一步研究及临床应用。Objective To investigate the influence of immune function and tumor recurrence of hyperbaric oxygen for colorectal cancer after Miles surgery. Methods 210 colorectal cancer patients accepting Miles surgery were enrolled in the study and randomly divided into the hyperbaric oxygen group（ n =106） and the control group（ n = 104）. The patients in the hyperbaric oxygen group used hyperbaric oxygen therapy at 1 d before operation,postoperative 3,4,5,6,7 d on the basis of parenteral nutrition and wound dressing. The patients in the control group accepted parenteral nutrition and wound dressing. The contents of interleukin-4（ IL-4） and interferon-γ（ INF-γ） on peripheral 1 d before surgery and 7 d after surgery were dected. Positron emission tomography computed tomography（ PET / CT） at 6 mo after surgery were used to observe the tumor recurrence. Results The IL-2 and INF-γ in the hyperbaric oxygen group was significantly increased than those of the control group（ P〈0. 05）,while the IL-4 in the hyperbaric oxygen group was significantly reduced than that of the control group（ P〈0. 05）. Compared with before treatment,IL-2、IL-4 and INF-γ after treatment were significantly changed in the hyperbaric oxygen group（ P〈0. 05）,IL-4 and INF-γ after treatment were significantly changed in the control group（ P〈0. 05）. The recurrence rate of the hyperbaric oxygen group was significantly lower than that of the control group（ P〈0. 05）.Conclusion Hperbaric oxygen treatment can enhance cellular immune function after surgery for rectal cancer using Miles surgery,prevent the recurrence of colorectal cancer effectively after 6 mo. And it deserves further research and clinical application.
