摘　　要：目的探讨腹腔镜手术治疗中期妊娠合并卵巢良性肿瘤的安全性与可行性。方法回顾性分析郑州大学第一附属医院行手术治疗的27例中期妊娠（13~28周）合并卵巢良性肿瘤患者的临床资料（腹腔镜手术13例,开腹手术14例）。结果腹腔镜手术组出血量少于开腹手术组[（16±6）ml vs（39±25）ml,P=0.004],切口疼痛评分、术后肛门排气时间、住院时间及术后盆腔粘连发生率均小于开腹手术组（P〈0.05）,两组患者新生儿体重及Apgar评分未见明显差异（P〉0.05）。结论腹腔镜手术治疗中期妊娠合并卵巢良性肿瘤安全可行。Objective To evaluate the safety and feasibility of laparoscopy in the management of benign ovarian tumors during the second trimester of pregnancy.Methods Data of twenty-seven patients with benign ovarian tumors during the second trimester of pregnancy who were treated in the First Affiliated Hospital of Zhengzhou University between 2012 and 2014were retrospectively analyzed.Laparoscopic management was performed in 13 patients whereas 14 underwent laparotomy.Results The amount of blood loss in the laparoscopy group was comparatively less[（16±6）ml vs（39±25）ml,P=0.004]and they had shorter postoperative hospital stay and lower postoperative pain score compared to the laparotomy group（P〈0.05）.The duration of the surgery in the two groups however were not significantly different.The pregnancy outcome of the two groups did not show significant differences in terms of gestational age at delivery,Apgar scores and birth weight（P〉0.05）.5patients in the laparoscopy group and 6patients in the laparotomy group delivered by cesarean section.Severe adhesions was found in the laparotomy group（χ~2=8.000;P=0.046）.Conclusions The results suggest that laparoscopic approach is safe in the management of benign ovarian tumors with significantly better surgical outcome than laparotomy.Laparoscopic approach is a better option for the management of benign ovarian cyst over laparotomy even during the second trimester of pregnancy.
