真人养脏汤治疗溃疡性结肠炎的系统评价

摘　　要：目的:评价真人养脏汤治疗溃疡性结肠炎的临床疗效。方法:检索EMBASE、PUBMED、MEDLINE、AMED、CINAHL、Cochrane图书馆、中国知网数据库、重庆维普中文科技期刊数据库、中国生物医学文献数据库、万方数据库和中国中医药期刊文献数据库从建库至2015年1月30日真人养脏汤治疗溃疡性结肠炎随机对照试验研究,筛选合格研究。应用Cochrane评价员手册进行偏倚风险评价,Jadad评分法进行质量评价。应用Rev Man5.3软件进行Meta分析。结果:符合纳入标准的文献共3篇。Meta分析结果显示,真人养脏汤治疗溃疡性结肠炎治疗组与对照组相比,总有效率比值比(odds ratio,OR)为4.97,95%可信区间(confidence interval,CI)[1.73,14.33],差异有统计学意义(P=0.003);近期治愈率OR为3.51,95%CI[1.92,6.42],差异有统计学意义(P<0.0001);纳入研究未报道不良反应。结论:真人养脏汤治疗溃疡性结肠炎临床疗效显著,但由于本系统评价纳人研究的总体质量较低,研究的数量和样本量均较小,尚需要更多高质量、多中心、大样本、严格设计的随机对试验来进一步验证其疗效与安全性。Objective： To evaluate the clinical efficacy of Zhenren Yangzang Decoction in the treatment of ulcerative colitis.Methods： EMBASE,PUBMED,MEDLINE,AMED,CINAHL,Cochrane Library,China National Knowledge Infrastructure Database,Chongqing VIP Chinese Science and Technology Periodical Database,Chinese Biomedical Literature Database,Wanfang Database and Traditional Chinese Medical Periodical Literature Database from the inception of each database up to 30 January 2015 were electronically searched. Randomized clinical trials（ RCTs） of Zhenren Yangzang Decoction for ulcerative colitis were included. The Cochrane risk of bias tool was used to assess the risk of bias. We assessed the quality of included studies using the Jadad rating scale. The Cochrane Collaboration＇s Rev Man5. 3 software was used for Meta- analysis. Results： Three studies were included and made a Meta- analysis. The Meta- analysis results showed that comparing with control group,Zhenren Yangzang Decoction treatment group＇s clinical total efficiency rate odds ratio（ OR） was 4. 97,95% confidence interval（ CI） [1. 73,14. 33] and the difference was statistically significant（ P = 0. 008）. The short- term cure rate OR was 3. 51,95% CI [1. 92,6. 42] and the difference was statistically significant（ P〈 0. 0001）. All studies included did not report adverse reactions. Conclusion： Zhenren Yangzang Decoction in the treatment of ulcerative colitis has significant clinical curative effect. However,the number and the study sample size are small and the methodological quality is low. To further evaluate the effect and safety of Zhenren Yangzang Decoction for ulcerative colitis,more rationally designed and strictly executed RCTs with large samples need to be made.
