超声引导微创旋切系统在乳腺良性肿瘤治疗中的应用

摘　　要：目的探讨超声引导微创旋切系统与常规开放手术在治疗乳腺良性肿瘤中的效果。方法将2014年9月~2015年8月我院收治的328例乳腺良性肿瘤患者根据手术方案分为研究组150例和对照组178例,研究组实施超声引导下的微创旋切手术,对照组则实施常规开放手术,术后随访1个月,比较两组患者的手术时间、术中出血量、切口长度、愈合时间、术后瘢痕大小等,并采用调查问卷了解患者的手术满意度。结果研究组和对照组的手术切除率分别为98.0%和99.4%,差异无统计学意义（P〉0.05）;研究组患者的手术时间、术中出血量、切口长度、愈合时间、术后瘢痕长度均显著低于对照组,差异有统计学意义（P〈0.05）;研究组术后并发症发生率为1.3%,显著低于对照组的5.1%（P〈0.05）;随访满意度调查结果显示,研究组总体满意度为96.0%,显著高于对照组的79.2%（P〈0.05）。结论超声引导下的麦默通微创旋切系统用于乳腺良性肿瘤的治疗具有创伤小、术后恢复快、安全性高且手术美观程度好的特点,比常规开放手术更具优势,值得在临床上推广和应用。Objective To investigate the efficacy of ultrasound guided minimally invasive rotary system compared with routine open surgeries in the treatment of benign breast tumor. Methods 328 patients with benign breast tumor who were admitted in our hospital form Sep 2014 to Aug 2015 were selected and assigned into research group of 150 patients and control group of 178 patients. Ultrasound guided minimally invasive rotary surgeries were adopted in research group while routine open surgeries were applied in control group. Postoperative follow-up was up to one year.Compare the operation time, intraoperative blood loss, length of incision, healing time and postoperative scar size and questionnaires were used to explore the satisfaction level of patients on the operations. Results The incision rate of research group and control group was 98.0% and 99.4% respectively with no significant difference（P〉0.05）; The operation time, intraoperative blood loss, length of incision, healing time and postoperative scar size of research group were significantly lower than those of control group and the difference showed statistical significance（P〈0.05）; The postopera tive complication rate was 1.3% and significant lower than the control group of 5.1%（P〈0.05）; The investigation result of satisfaction by follow-up showed that the general satisfaction in research group was 96.0%, significantly higher than that in control group with 79.2%（P〈0.05）. Conclusion The application of ultrasound guided Mammotome minimally invasive rotary system in the treatment of benign breast tumor has the character of less trauma, quicker healing, higher safety and better operative appearance, which is more advanced compared with routine open surgeries and deserves clinical practice and application.
