表皮生长因子受体在人胃癌组织中的表达及其与预后的相关性研究

摘　　要：目的：探讨EGFR在人胃癌中的表达及其与预后的相关性。方法选择2008年7月至2011年5月河南大学第一附属医院收治的有随访结果且资料完整的98例胃癌患者，采用免疫组化法对EGFR蛋白在胃癌组织与胃正常组织中的表达情况进行检测分析。分析EGFR蛋白在胃癌中各级表达情况、EGFR蛋白与临床病理学特征之间的关系、EGFR蛋白与患者预后之间的相关性。结果（1）EGFR蛋白各级分布情况如下：0级为56.12%（55/98），1级为23.47%（23/98），2级为11.22%（11/98），3级为9.18%（9/98）；（2）EGFR表达与患者性别、年龄、肿瘤大小、肿瘤发生部位、分化程度方面无相关性（P〉0.05），但是与肿瘤浸润深度、是否有淋巴结转移、是否发生远处转移、TNM分期方面存在显著相关性（P〈0.05）；（3）EGFR与患者预后呈显著相关性（P=0.019，r=0.66），EGFR阳性患者平均生存时间为22.76个月，阴性患者平均生存时间为39.39个月，两组比较差异有统计学意义（P〈0.05）。结论 EGFR蛋白在人胃癌各级组织中均有表达，且与患者预后存在显著的相关性，可作为临床判断胃癌的一个重要指标。ObjectiveThe aim of this study was to evaluate the EGFR expression in the gastric cancer patients and associated with prognosis.Methods Ninety eight patients with gastric cancer with completed clinical data were retrospectively analyzed in our study.The EGFR expression in gastric cancer tissue were tested by immunohistochemical array.The EGFR expression model and association with clinical characteristics and prognosis were evaluated.Results （1） The EGFR expression model was：0 grade 56.12%（55/98）,1grade 23.47%（23/98）,2 grade 11.22%（11/98）,3 grade 9.18%（9/98）;（2） There was no association between sex,gender,tumor diameter,tumor location tumor metastasis and EGFR expression（P〉0.05）;（3） Significant association was found between EGFR expression and prognosis（P〈0.05）;The median survival was 22.76 in positive and 39.39 moths in negative group with statistical group（P〈0.05）. Conclusion EGFR protein in human gastric cancer are expressed in the all levels, and there is significant correlation with the prognosis of patients with can be used as an important prognosis indicator of gastric cancer.
