重度牙周炎临床基础治疗与牙髓治疗效果分析

摘　　要：目的探讨慢性牙周炎症对牙髓感染的影响及重度牙周炎患牙的牙髓治疗时机和方案。方法将443例重度牙周炎患牙分为基础治疗观察组及牙髓敏感干预组,牙周基础治疗后,牙髓敏感患者40例共计108颗患牙进行根管治疗,经6个月随访,通过对比分析基础治疗前、后(1、3、6个月)的牙髓活力检查(冷测、电测)及牙周临床检查结果进行疗效评估。结果患牙牙髓敏感度经牙周基础治疗一个月后较治疗前有所增加,至治疗结束(6个月随访)患牙牙髓敏感度较治疗前无明显下降。根管治疗对于患牙牙周探诊出血指数(bleeding on probing,BOP)、牙周袋深度(probing depth,PD)及临床附着丧失(clinical attachment loss,CAL)指标较非根管治疗组治疗后比较无统计学差异。结论基础治疗对于牙髓敏感症状的缓解作用并不明显;而根管治疗对于改善重度慢性牙周炎患牙的牙周基础状况作用亦不明显。Objective To compare the efficacy of basic treatment with pulp treatment for sever periodontitis. Method One hundred thirty eight patients with severe periodontitis（ 443 teeth） patients were divided into basic treatment and pulp sensitive intervention groups. Forty pulp sensitive patients（ 108 teeth） after periodontal basic treatment received pulp treatment. After six months followup visits,the pulp test（ cold,electrical measurement） and clinical periodontal index before and 1,3,6 month after treatment were observed and compared between two groups. Result The pulp sensitivity was improved after 1 month of periodental non-surgical treatment,by the end of treatment there was no significant difference in teeth sensitivity between two groups. There were no significant differences in periodontal examination index,including bleeding on probing（ BOP）,probing depth（ PD） and clinical attachment loss（ CAL） before and after root canal treatment in two groups. Conclusion Basic treatment shows no significant alleviated effects in pulp sensitivity,and root canal treatment provides no significant improvement for periodontal condition of patients with sever periodontitis.
