止血散、清肠汤、冠心胶囊联合西药治疗肝硬化门脉高压上消化道出血随机平行对照研究

摘　　要：[目的]观察止血散、清肠汤联合西药治疗肝硬化门脉高压上消化道出血疗效。[方法]使用随机平行对照方法,将82例住院及门诊患者按抛硬币法简单随机分为两组。对照组41例吸氧、补充血容量、抑酸、保肝、预防感染常规治疗;亚硫酸氢钠甲萘醌4mg/次,3次/d,肌注;氨甲苯酸0.2g+0.9%生理盐水20m L,1次/d,静滴;西咪替丁0.6g+0.9%生理盐水3m L,1次/d,静滴;去甲肾上腺素2mg+0.9%生理盐水20m L,3次/d,口服;冠心胶囊,2g/次,3次/d。治疗组41例止血散(白及粉、三七粉、大黄、碳粉炒各5g),100m L凉水米汤冲服,8h/次;清肠汤(蒲公英3g,赤芍、乌梅各30g,黄连、生牡蛎、生槐米各15g,生大黄5g),1剂/d,水煎400m L,早晚口服,200m L/次;西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、血压、血红蛋白浓度、红细胞、出血量、不良反应。治疗1疗程,判断疗效。[结果]治疗组显效15例,有效23例,无效3例,总有效率92.68%。对照组显效12例,有效19例,无效10例,总有效率75.61%。治疗组疗效优于对照组(P<0.05)。[结论]止血散、清肠汤联合西药治疗肝硬化门脉高压上消化道出血疗效满意,无严重不良反应,值得推广。[Objective]To observe the scattered bleeding,bowel soup combined with Western medicine treatment of gastrointestinal hemorrhage on portal hypertension.[Methods]Randomized parallel controlled method,82 inpatients and outpatients law simply by flipping a coin randomly divided into two groups.The control group 41 cases of oxygen,to add volume,acid suppression,liver,prevention of infection conventional treatment;sodium bisulfite menadione,4mg/time,3 times/d,intramuscular injection;amino benzoic acid 0.2g＋20m L 0.9% physiological salt,1 times/d,intravenous infusion;cimetidine 0.6g＋3m L 0.9% saline,1 times/d,intravenous infusion;norepinephrine 2mg ＋ 20 m L 0.9% saline,3 times/d,orally;CHD Capsules,2g/time,3 times/d.The treatment group 41 cases bleeding powder（Baiji,Sanqi,Dahuang,Tanfen each 5g）,100 m L cold soup blunt,8h/second;bowel soup（Pugongying 3g,Chishao,Wumei each 30 g,Huanglian,Shengmuli,Huaimi the 15 g,Shengdahuang 5g）,1 ji/d,decoction 400 m L,sooner or later,oral,200 m L/times,even serving eight weeks.Western medicine with the control group.7d for a course of continuous treatment.Observation of clinical symptoms,blood pressure,hemoglobin concentration,red blood cells,the amount of bleeding,adverse reactions.One course of treatment,to determine efficacy.[Results]The treatment group 15 cases,effective 23 cases,3 cases,the total efficiency of 92.68%.The control group 12 cases markedly effective in 19 cases,10 cases,the total efficiency of 75.61%.Treatment group than the control group（P〈0.05）.[Conclusion]San bleeding,bowel soup combined with Western medicine treatment of upper gastrointestinal hemorrhage with cirrhosis and portal hypertension satisfaction,no serious adverse reactions,is worth promoting.
