摘 要:目的探讨USCOM心输出量超声监测仪对心脏骤停患者心脏复苏的临床效果。方法回顾性分析研究2012年1月至2015年11月心脏停博患者,对照组采用Thumper型心肺复苏机胸外心脏按压,按压深度5cm,试验组也采用Thumper型心肺复苏机进行胸外心脏按压,按压深度根据USCOM心输出量超声监测仪的心输出量进行调节,两组均按照《2015年国际心肺复苏和心血管急救指南》进行急救,比较两组患者心脏复苏成功率、复苏成功时间、有创动脉血压、动脉血氧分压、动脉血氧饱和度、肋骨骨折发生率。结果试验组心脏复苏成功率、有创动脉血压、动脉血氧分压、血氧饱和度明显优于对照组,差异具有统计学意义(均P〈0.05);肋骨骨折发生率、心脏复苏成功时间明显低于对照组,差异具有统计学意义(均P〈0.05)。结论心脏骤停患者Thumper型心肺复苏机进行胸外心脏挤压,通过采用USCOM心输出量超声监测仪调节胸外心脏按压深度,能达到理想的心脏按压效果,显著提高心脏复苏成功率,降低并发症的发生。Objective To investigate clinical effects of the USCOM ultrasonic cardiac output monitor on cardiac arrest cardiac resuscitation. Methods In this retrospective analysis from January 2012 to November 2015, cardiac arrest patients of the control group received Thumper type CPR machine chest compressions(compression depth 5 cm), while the experimental group also uses Thumper type CPR chest compressions machine (compression depth adjusted according to cardiac output USCOM ultrasonic cardiac output monitor). The success rate, successful resuscitation time, invasive arterial blood pressure, blood pressure, arterial oxygen saturation, and rib fracture incidence were compared between the two groups. Results The results of the test group of cardiac resuscitation success rate, invasive arterial blood pressure, blood pressure, oxygen saturation were significantly better than the control group ( all P 〈 0.05). The incidence of rib fractures and cardiac resuscitation time was significantly lower than the control group(beth P 〈 0.05). Conclusion Thumper type of cardiac arrest CPR machine chest cardiac compression by using USCOM ultrasonic cardiac output monitor to adjust the depth of chest compressions, cardiac massage can achieve the desired effect, can significantly improve the success rate of cardiac resuscitation, reduce complications.
