摘 要:目的探讨光动力疗法(photodynamic therapy,PDT)联合局部化疗对小鼠H22肝癌移植瘤的抗肿瘤作用及其机制。方法昆明小鼠40只,皮下接种H22肝癌细胞建立荷瘤鼠模型。随机分为4组,每组10只小鼠。分别为对照组(C组)、氟尿嘧啶植入剂组(SF组)、光动力治疗组(PDT组)和联合治疗组(PDT+SF组)。治疗后定期测量各组小鼠种植瘤体积;处理后第14天,每组处死5只荷瘤小鼠,称取瘤重,计算抑瘤率;观察HE染色的瘤组织病理学改变;流式细胞术检测细胞凋亡及细胞周期分布情况;剩余5只荷瘤小鼠观察生存期,计算生命延长率。结果 (1)处理14 d后,各处理组的肿瘤体积平均值小于空白对照组,以PDT+SF组肿瘤体积平均值最小(P<0.05);(2)SF、PDT、PDT+SF组抑瘤率分别为:50.37%、43.73%、68.27%;(3)治疗后各处理组均出现肿瘤细胞数量减少、病理性核分裂象减少,可见片状坏死及空泡样变性,上述病理改变以PDT+SF组更为显著。(4)处理后14 d,各处理组肿瘤细胞凋亡率均高于对照组,以PDT+SF组凋亡率最高;与对照组相比,各处理组G0/G1期细胞所占比例均升高,S期细胞所占比例均下降,以PDT+SF组变化最为显著。(5)各处理组均能延长小鼠生存时间,其中PDT+SF组小鼠平均生存时间最长、生命延长率最高(P<0.05)。结论光动力治疗与氟尿嘧啶植入剂联合应用能够更有效地抑制肿瘤生长,促进细胞凋亡,延长荷瘤小鼠生存期。Objective To investigate the anti-tumor effect of combined application of photodynamic therapy and fluorouracil implant on mice H22 transplanted hepatoma,and explore the antitumor mechanisms.Methods Totally 40 Kunming mice with H22 hepatoma were divided into four groups randomly: the control group( C),fluorouracil implant group( SF),photodynamic therapy group( PDT) and combination therapy group( PDT + SF). The mice in Group C were not given any treatment. While the mice in Group SF being implanted 100 mg / Kg fluorouracil implants intratumor,those in Group PDT were given PDT. In Group PDT + SF,the mice were given implantation of 100 mg / kg fluorouracil implants intratumor plus PDT. For each group,the tumor size was measured every other day,5 mice were killed 14 days later and the tumor suppressive rate was calculated. The pathological sections of the tumor tissues were observed with HE staining,and the apoptosis and cell cycle distribution of tumor were detected with flow cytometer. The survival time was observed and life extension rate calculated.Results( 1) The tumor size was smaller in the treatment groups than in the control group on the 14 th day after the therapy. As to the three treatment groups,the tumor size in Group PDT + SF was the smallest( P 0. 05).( 2) The tumor inhibition rates of Groups C,SF,PDT and PDT + SF were 50. 37%,43. 73% and 68. 27% respectively.( 3) For all the treatment groups,the number of tumor cells and pathological mitosis reduced,and necrosis and vacuolar degeneration occurred. Of these groups,Group PDT + SF boasted the largest necrosis area.( 4) Compared with the control group,all the treatment groups had higher apoptosis rates. Among them,Group PDT + SF got the highest apoptosis rate. While the proportions of G0 / G1 phase cells in Group SF,Group PDT and Group PDT+ SF being growing,those of S phase cells declined.
