摘  要:目的采用Meta分析评价我国单独使用半导体激光治疗牙本质过敏症的临床疗效。方法计算机检索中文生物医学文献数据库、中国期刊全文数据库、维普数据库、万方数据库等,筛选国内有关使用半导体激光治疗牙本质过敏的随机对照临床研究,检索范围为1997~2014年。在严格筛选资料和质量评价的基础上,采用Rev Man5.3软件对纳入的研究结果进行Meta分析。结果共纳入8篇临床对照试验研究。Meta分析结果显示:对于半导体激光治疗牙本质敏感症1周和1个月后的疗效数据采用固定模型分析。治疗即刻和6个月后的疗效数据存在统计学异质性,采用随机模型分析。半导体激光治疗牙本质敏感的1周和1个月疗效优于对照组(使用75%氟化钠甘油治疗),差异有统计学意义(1周疗效:OR合并=10.13,95%CI=4.80~21.38;1个月疗效:OR合并=0.25,95%CI=0.19~0.31)。即刻疗效和6个月疗效有优于对照组的趋势,差异有统计学意义(即刻疗效:OR合并=4.29,95%CI=2.13~8.64;6个月疗效:OR合并=6.27,95%CI=1.53~25.67)。结论使用半导体激光治疗牙本质过敏疗效较75%氟化钠甘油好。但由于本研究中部分文献的质量不高,上述结论还需要更多设计严谨且样本量足够的随机临床试验进一步验证。Objective This meta-analysis was to assess the efficacy of diode lasers in reducing dentin hypersensitivity (DH) as compared with the efficacy of 75% sodium fluoride glycerin in China. Methods A search of literatures about clinical efficacy of diode lasers in reducing dentin hypersensitivity in CMCC, CNKI, VIP, Wanfang databases was conducted from 1997 to 2014 . The quality of included studies was stringently evaluated and data were analyzed by the RevMan 5.3 software. Results After selection, 8 studies were included in the meta-analysis. Meta- analysis results showed that 1 week and 1 month efficiency for the treatment of dentine hypersensitivity using diode lasers were higher than the control group (75% sodium fluoride glycerin). The differences were statistically significant (1 week efficiency: OR =10.13, 95%CI =4.80-21.38; 1 month efficiency: OR =0.25, 95%CI =0.19-0.31). Immediate and 6 months efficiency might be superior to the control group (immediate efficiency : OR=4.429, 95%CI=2.13-8.64; 6 months : OR =6.27, 95%CI =1.53-25.67). Conclusion The desensitization of dentin hypersensitivity with diode lasers is more effective than 75% sodium fluoride glycerin.However, given the low quality of some included evidences, more well-planned randomized clinical trials with adequate sample size are needed to confirm the results.
