多层螺旋CT评价胰腺癌胰周淋巴结转移的价值

摘　　要：目的:探讨多层螺旋CT(MSCT)在诊断胰腺癌胰周淋巴结转移中的价值。方法:回顾性分析11例手术病理确诊胰腺癌伴胰周淋巴结转移的患者,对其胰周淋巴结的大小、发生部位、形态以及强化后表现等进行观察分析,并与病理结果比较,得出MSCT对胰周淋巴结转移诊断的灵敏度、特异度、一致率以及Youden指数。结果:淋巴结转移以腹腔动脉干组最多,肠系膜根部组和腹主动脉周围组其次,肝十二指肠韧带组最少。MSCT对胰腺癌胰周淋巴结转移诊断的灵敏度为95.8%,特异度为75.0%,一致率为90.6%,Youden指数为0.71。结论:MSCT对胰腺癌胰周淋巴结的转移有较高的准确率,对胰腺癌治疗前的评估具有非常重要的意义。Objective： To discuss the value of MSCT in diagnosing lymph node metastases of pancreatic carcinoma. Methods： Eleven cases pancreatic carcinoma and lymph node metastases with were confirmed by operative pathology,the size,location,morphology,enhancement of lymph nodes were observated and analyzed,compared with pathological results,the sensitivity,specific,consistent rate and Youden index of MSCT in diagnosing lymph node metastases of pancreatic carcinoma were concluded. Results： Lymph node metastases were most seen in celiac trunk,the superior mesenteric and para- aortic area secondly,the hepar- duodenum area at least. The sensitivity MSCT of lymph node metastasis of pancreatic cancer diagnosis was95. 8%,the specific degree was 75. 0%,the consistent rate was 90. 6%,Youden index is 0. 71. Conclusion： It was a high accuracy of MSCT in diagnosing the lymph node metastases of pancreatic carcinoma,and was important to evaluate the pancreatic carcinoma before treatment.
