摘　　要：利用医院现有的医疗保险网络,与卫生局、医保局联合构建分级诊疗机制和分级诊疗支持与监管平台,利用信息化手段构建分级诊疗平台,实现区域内医疗机构的患者互转、就医信息共享、远程预约及远程会诊、患者院后管理等功能,是医疗机构、医疗保险管理部门管理跨医疗机构患者就医的有效工具,能够实现分级诊疗中患者管理的闭合环路,对于降低患者费用、缓解患者看病难等问题有较大作用。Utilizing hospital＇s existing health insurance network,cooperating with the Public Health Bureau and the Health Insurance Bureau to build Grading diagnosis mechanism and medical support and monitoring platform. Utilizing information technology approaches to build classification diagnosis platform,to achieve patient transfer within regional medical institutions,medical information sharing,remote reservation,remote consultation,and other functions such like post-hospitalization patients management. An efficient tool for medical institutions and health insurance administration to manage across medical institutions patients. Can achieve classification diagnosis and treatment of patients with closed loop management. Have a greater effect for reducing patient costs,remission in patients with difficulties to get medical services.
