摘  要:目的分析甲状腺微小癌(TMC)进行超声弹性成像检查的诊断价值。方法甲状腺占位性病变患者200例,进行二维常规超声检查,筛查出甲状腺实性结节150个,直径均≤10 mm,进行超声弹性成像检查,金标准为病理检查结果,评估常规超声与超声弹性成像的特异性及灵敏度。结果病理诊断结果显示良性结节103个,恶性结节47个;超声弹性成像检查良性结节99个,恶性结节51个;二维常规超声检查良性结节97个,恶性结节53个;超声弹性成像、二维常规超声的阳性预测值分别为80.4%、56.6%,差异有统计学意义(P<0.05);超声弹性成像、二维常规超声的灵敏度分别为87.2%、63.8%,差异有统计学意义(P<0.05);超声弹性成像、二维常规超声的特异度分别为90.3%、77.7%,差异有统计学意义(P<0.05)。结论直径≤10 mm的甲状腺癌进行超声弹性成像检查,灵敏度及特异度较高,可作为甲状腺微小癌的首选检查方式,值得临床推广。Objective To analyze the diagnostic value of ultrasonic elastography for TMC. Methods 200 patients with thyroid space-occupying lesions treated received routine ultrasonic examination. There were 150 solid thyroid nodules( dia. ≤10mm). Through ultrasonic elastography,the gold standard was pathological examination result. The specificity and sensitivity based on routine ultrasonic examination and ultrasonic elastography were evaluated. Results According to pathological diagnosis,there were 103 benign nodules and 47 malignant nodules; based on ultrasonic elastography,there were 99 benign nodules and 51 malignant nodules; based on routine ultrasonic examination,there were 97 benign nodules and 53 malignant nodules. The positive predictive values based on ultrasonic elastography and routine ultrasonic examination were 80. 4% and 56. 6%( P〈 0. 05); the sensitivity based on ultrasonic elastography and routine ultrasonic examination were 87. 2% and 63. 8%( P〈 0. 05); the specificity based on ultrasonic elastography and routine ultrasonic examination were 90. 3% and 77. 7%( P〈 0. 05). Conclusion For thyroid carcinoma( dia. ≤10 mm),ultrasonic elastography has a higher sensitivity and specificity and can be regarded as first choice for examination of TMC. It is worthy of clinical promotion.
