不同术式在大肠肿瘤手术中的应用效果及对机体免疫功能的影响

摘　　要：目的比较不同术式在大肠肿瘤手术中的应用效果及其对机体免疫功能的影响。方法随机将患者分成腹腔镜组和开腹组,每组各20例。比较不同术式在手术时间、出血量、排气时间、标本长度、淋巴总数等方面的治疗及根治效果;通过ELISA方法测定血清中Ig A、Ig M、Ig G、C-反应蛋白(C-reactive protein,CRP)含量,比较不同术式对机体免疫功能的影响。结果腹腔镜组和开腹组手术出血量分别为(186.9±79.8)ml、(230.6±68.3)ml;手术排气时间分别为(46.7±9.5)s、(63.9±13.2)s;Ig M水平分别为(0.99±0.41)g·L^(-1)、(0.68±0.16)g·L^(-1);CRP水平分别为(43.95±12.05)mg·L^(-1)、(59.01±10.55)mg·L^(-1)。2组比较均具有统计学差异(P<0.05)。结论腹腔镜组和开腹组在手术时间、切除肠管长度和清除淋巴数目方面差异无统计学意义,但在手术出血量、术后排气时间以及Ig M、CRP机体免疫指标方面,腹腔镜组优于开腹手术组,腹腔镜手术在临床上值得推广。Objective To compare the effect of different surgical applications in colorectal cancer surgery and its impact on the body＇s immunity. Methods The patients were randomly divided into the laparoscopic group and the open surgery group,20 cases in each. Surgical operative time,blood loss treatment and curative effect,exhaust time,length of the specimen,total lymph number in different surgical methods were compared; serum Ig A,Ig M,Ig G,C-reactive protein（ C-reactive protein,CRP） content were measured by ELISA method,and effect of different surgical methods on immune function were compared. Results Bood loss in the laparoscopic group and the open surgery group were（ 186. 9 ± 79. 8） ml,（ 230. 6 ± 68. 3） ml; surgery exhaust time were（ 46. 7 ± 9. 5） s,（ 63. 9 ± 13. 2） s; Ig M levels were respectively（ 0. 99 ± 0. 41） g·L^- 1,（ 0. 68 ± 0. 16） g·L^- 1; CRP levels were（ 43. 95 ± 12. 05） mg·L^- 1,（ 59. 01 ± 10. 55） mg·L^- 1. There had significant difference（ P〈 0. 05）. Conclusion Laparoscopy and laparotomy group in operation time,removed intestinal canal length and lymph dissection number have no statistically significant differences,but in surgical blood loss,postoperative exhaust time and Ig M,CRP immune indicators,laparoscopic group is better than that of the open surgery group,laparoscopic surgery is worth promoting in clinic.
