核基质蛋白22和膀胱肿瘤抗原对国人膀胱癌诊断价值的系统评价

摘　　要：目的：系统评价并比较尿液中核基质蛋白22（NMP22）和膀胱肿瘤抗原（BTA stat）对国人膀胱癌的临床诊断效能。方法：利用计算机检索PubMed、MEDLINE、Cochrane Library、Web of Science、万方数据库、中国知网（CNKI）等中英文数据库,按照纳入和排除标准收集2000年1月-2015年5月发表的所有比较NMP22和BTA stat对膀胱癌诊断效能的文献,对纳入研究进行数据提取和质量评价,采用Cochrane协作网提供的Revman5.3、Meta-Di Sc 1.4两种软件进行Meta分析。结果：共纳入7篇文献,总样本量837例,其中膀胱癌患者476例,非膀胱癌361例。NMP22与BTA stat汇总敏感度分别为0.84（95%CI：0.80-0.87）和0.75（95%CI：0.71-0.79）,汇总特异度分别为0.83（95%CI：0.79-0.87）和0.75（CI：0.70-0.79）,Q指数分别为0.8 276和0.7824,综合受试者工作特征曲线下面积分别为0.8 965和0.8 512,Z检验结果显示差异无统计学意义（P〉0.05）。结论：对于国人膀胱癌的诊断,NMP22与BTA stat相比,前者敏感度和特异度均稍高;二者在高级别和浸润性膀胱癌中的阳性表达率均高于低级别和非肌层浸润性膀胱癌;二者诊断效能均较高,但差异无统计学意义。Objective：To systematically evaluate the value of urine nuclear matrix protein 22（NMP22）and bladder tumor antigen stat（BTA stat）in the diagnosis of bladder cancer in Chinese.Method：According to the requirements of meta-analysis,PubMed,MEDLINE,Cochrane Library,Web of Science,Wan Fang Data and CNKI were searched for studies about NMP22 and BTA stat in the diagnosis of bladder cancer in Chinese from January2000 to May 2015.After collection and evaluation of data,a meta-analysis was made by Revman 5.3and MetaDiSc 1.4.Result：Seven studies were included within 837 patients and in whom 476 were bladder cancer cases and361 were not.Pooled sensitivity and specificity of NMP22 were slightly higher than those of BTA stat（0.84,95%CI：0.80-0.87 vs 0.75,95%CI：0.71-0.79）and（0.83,95%CI：0.79-0.87 vs 0.75,95%CI：0.70-0.79）,respectively.Q index of NMP22 and BTA stat were 0.8 276 and 0.7 824.We drawed the summary receiver operating characteristic（SROC）and found the area under curve（AUC）of NMP22 was a little bigger than that of BTA stat（0.8965 vs 0.8512）.However,no significant difference was found by Z-test（P〉0.05）.Conclusion：In terms of diagnosis of bladder cancer in Chinese,the sensitivity and specificity of NMP22 are slightly higher than those of BTA stat.Besides,the sensitivity of those two tests in diagnosis of bladder cancer are higher in high grade and muscle-invasive bladder cancer（MIBC）than in low grade and non-muscle invasive bladder cancer（NMIBC）.The diagnosis value of NMP22 and BTA stat in bladder cancer are both high,but no significant difference can be found.
