消痞萎胃康颗粒治疗慢性萎缩性胃炎70例临床研究

摘　　要：[目的]观察消痞萎胃康颗粒治疗慢性萎缩性胃炎伴肠上皮化生及非典型增生(CAG+IM+ATP)(属于中医肝胃不和、脾虚瘀滞型)的临床疗效。[方法]将100例CAG及伴有IM、ATP患者分为治疗组70例和对照组30例。治疗组给予消痞萎胃康颗粒口服,每次20克,每日3次。对照组给予摩洛丹,每次1丸,每日3次。2组均按病理分级轻度治疗3个月,中度治疗4个月,重度治疗6个月。观察治疗前后2组患者临床症状、临床疗效、胃镜及胃黏膜组织变化。[结果]治疗组临床疗效总有效率为92.86%,对照组为66.67%,治疗组明显优于对照组(P<0.05);2组患者治疗后症状积分明显下降(P<0.05);治疗组病理改善率明显优于对照组(P<0.05);治疗组治疗肠化总有效率为65.12%,对照组为37.50%,治疗组优于对照组(P<0.05);治疗组治疗非典型增生总有效率为60.00%,对照组样本不足,未做统计;治疗组治疗腺体萎缩总有效率为69.12%,对照组为64.29%,差异无统计学意义(P>0.05);治疗组Hp根除率为52.78%,对照组Hp根除率为52.94%,治疗组与对照组疗效相当(P>0.05)。治疗后治疗组糜烂,黏膜白相、颗粒增生、血管透见、胆汁反流总有效率明显优于对照组(P<0.05)。[结论]消痞萎胃康颗粒能有效改善CAG及伴有IM、ATP患者的临床症状和病理改变,改善胃黏膜血液循环,使黏膜再生加强并得到修复,同时使IM、ATP得以有效逆转,是治疗CAG及伴有IM、ATP的有效方药。[Objective]To observe the clinical efficacy of Xiaopiwei Weikang granule（XWG）on chronic atrophic gastritis（CAG）with intestinal metaplasia（IM）and atypical dysplasia（ATP）（CAG＋IM＋ATP）（belonging to the Traditional Chinese Medicine type of disharmony between liver and stomach,spleen deficiency and blood stasis）.[Methods]One-hundred patients with CAG and IM,ATP were divided into treatment group（70cases）and control group（30cases）.The treatment group was given XWG orally with 20 grams each time and 3times a day.The control group was given Moluo Dan with one pill each time and 3times a day.Both groups were given corresponding treatment according to pathological grading,3months treatment for mild,4 months treatment for moderate and 6 months treatment for severe.Clinical symptoms,clinical efficacy and the endoscopy and mucosal tissue changes of the patients in both groups were observed before and after the treatment.[Results]The total effective rate of the clinical efficacy in the treatment group was 92.86%,which was obviously higher than 66.67% in the control group（P〈0.05）.Symptom score of the patients in both groups decreased significantly after the treatment（P〈0.05）.Thepathology improvement rate of the treatment group was much higher than that of the control group（P〈0.05）;The total effective rate for intestina treatment group was 65.12%,which was significantly higher than 37.50%in the control group（P〈0.05）.The total effective rate of curing atypical hyperplasia in the treatment group was 60.00%,and there was no statistics in the control group because of lacking enough samples.The total effective rate of glandular atrophy in the treatment group was 69.12% and 64.29%in the control group,which showed no statistically significant difference（P〈0.05）.The Hp eradication rate of the treatment group was 52.78% and 52.94% in the control group.Curative effect of the treatment group and the control group was equal（P〈0.05）.The total efficiency of erosion,mucosal
