生长抑素联用丹参或乌司他丁治疗重症急性胰腺炎疗效观察

摘　　要：目的探讨生长抑素联用丹参或乌司他丁治疗重症急性胰腺炎(SAP)的效果。方法将246例SAP患者分成4组:生长抑素组(A组);生长抑素联合丹参组(B组);生长抑素联合乌司他丁组(C组);生长抑素联合乌司他丁和丹参组(D组)。观察4组患者腹痛及腹胀症状消失时间、心率及呼吸恢复正常时间、血淀粉酶及血糖恢复正常时间、住院时间、总住院费用及有效率。结果患者腹痛及腹胀消失时间、心率及呼吸恢复正常时间、血淀粉酶及血糖恢复正常时间和住院时间最短为D组,其他依次为C、B、A组,4组比较差异有统计学意义(P<0.05);A、B、C、D组总有效率分别为65.57%、77.42%、81.54%、89.66%,4组比较差异有统计学意义(P<0.05);4组不良反应发生率比较,差异无统计学意义(P>0.05)。结论生长抑素能有效治疗SAP,与乌司他丁或丹参联用能提高疗效,但乌司他丁的疗效优于丹参,乌司他丁与丹参联用能进一步提高疗效。Objective To investigate the effect of somatostatin combined with Salvia miltiorrhiza or ulinastatin in treating severe acute pancreatitis（SAP）.Methods Two hundred and forty-six patients with SAP were randomly assigned into 4groups：somatostatin group（A）,somatostatin＋ Salvia miltiorrhiza group（B）,somatostatin＋ ulinastatin（C）and somatostatin＋ Salvia miltiorrhiza＋ ulinastatin group（D）.The disappearance time of abdominal pain and distention symptoms,time of heart rate and respiration rate recovering to normality,time of blood amylase and blood glucose recovering to normality,hospitalization time,total hospitalization cost and effective rate were observed and compared among 4groups.Results The disappearance time of abdominal pain and distention symptoms,time of heart rate and respiration rate recovering to normality,time of blood amylase and blood glucose recovering to normality,and hospitalization time in the group D were shortest,followed by the group C,B and A in turn,the differences among them were statistically significant（P0.05）;the total effective rates in the group A,B,C and D were 65.57%,77.42%,81.54% and 89.66% respectively,and there were statistical significances among 4groups（P0.05）;there were no statistical difference in the occurrence rate of adverse reactions among 4groups（P0.05）.Conclusion Somatostatin is effective in the treatment of SAP,the combination use of ulinastatin or Salvia miltiorrhiza can improve the effect,but ulinastatin is superior to Salvia miltiorrhiza in the effect,their combination can further improve the therapeutic effect.
