摘 要:目的研究P16、P38、P53和CyclinD1蛋白在食道癌中表达的意义,为食道癌早期诊断提供依据。方法选用河北医科大学外科手术切除治疗的食管癌存档蜡块为实验标本共123例,将标本制成4μm厚病理切片后进行HE染色,采用免疫组合化学法检测病理切片中P16、P38、P53和CyclinD1蛋白的表达量。收集、整理123例患者临床资料,应用SPSS软件对相关数据进行分析,研究P16、P38、P53和CyclinD1蛋白与临床病理因素的关系。结果 123例食管癌患者组织中P16、P38、P53和CyclinD1阳性分别为59、64、44和36例,阳性率分别为47.97%、52.03%、35.8%和29.3%。统计分析显示,P16、P38、P53和CyclinD1蛋白表达与瘤组织侵润深度及淋巴结转移有相关性(P<0.05或P<0.01),吸烟组P53蛋白表达与吸烟量有相关性(P<0.05);P38、P53和CyclinD1蛋白表达与饮酒量有相关性(P<0.05),P53和CyclinD1蛋白表达与癌远处转移量有相关性(P<0.05)。讨论P16、P38、P53和CyclinD1的表达与食道癌的发生密切相关,可用于食道癌早期诊断和辅助治疗。Objective The expression of P16,P38,P53,and CyclinD1 in esophageal cancer was studied in order to provide evidence for early diagnosis of esophageal cancer. Methods Specimens were 123paraffin-embedded sections of esophageal carcinoma from patients who underwent surgical resection at the Fourth Hospital of Hebei Medical University.These paraffin-embedded sections were cut into 4-mm-thick pathology specimens and subjected to HE staining.The expression of P16,P38,P53,and CyclinD1 was detected immunohistochemically.Detailed clinical and pathological data on patients were collected,and relevant data were analyzed with SPSS to study the relationship between P16,P38,P53,and CyclinD1 and clinical and pathological factors in esophageal cancer. Result Expression of P16 was noted in 59patients(47.97%),expression of P38 was noted in 64(52.03%),expression of P53 was noted in 44(35.8%),and expression of CyclinD1 was noted in 36(29.3%).Statistical analysis indicated that P16,P38,P53,and CyclinD1 expression was related to the depth of tissue invasion and lymph node metastasis.Levels of P38,P53,and CyclinD1 expression were higher for more invasive cancer(T3+T4)than for less invasive cancer(Tis+T1+T2),though the same was not true for the level of P16 expression.The level of P53 expression was higher in patients with lymph node metastasis than in patients without that metastasis,though the same was not true for the level of P16 expression.The level of P53 expression was higher in smokers than in non-smokers,but the difference in that level of expression was not statistically significant.Levels of P38,P53,and CyclinD1 expression were related to the amount of alcohol consumption.The levels of P53 and CyclinD1expression were higher in patients with distant metastasis than in patients without distant metastasis. Conclusion Expression of P16,P38,P53,and CyclinD1 were closely related to esophageal cancer.This fact can be used for to diagnose esophageal cancer early on and provide adjuvant therapy.
